局部麻醉对老年腹股沟疝修补术患者免疫功能的影响

【摘要】：目的探讨局部麻醉对老年腹股沟疝修补术患者免疫功能的影响。方法 70例行无张力疝修补术治疗老年腹股沟疝患者随机分为局部麻醉组和连续硬膜外麻醉组,每组各35例,分别采用局部麻醉法和连续硬膜外麻醉下施行腹股沟疝无张力修补术,观察两组患者麻醉前1 d(T0)、术后1 d(T1)和术后3 d(T2)的白细胞(WBC)、淋巴细胞绝对值(LY)、白细胞介素6(IL-6)和肿瘤坏死因子α(TNF-α)水平。结果 T1时点WBC水平局部麻醉组明显低于连续硬膜外麻醉组(P0.05),LY局部麻醉组明显高于连续硬膜外麻醉组(P0.05),T2时点IL-6水平局部麻醉组明显高于连续硬膜外麻醉组(P0.05)。结论局部麻醉对老年腹股沟疝修补术患者主要免疫功能指标影响较小,可作为老年腹股沟疝无张力修补术的首选。
[Abstract]:Objective to investigate the effect of local anesthesia on the immune function of elderly patients undergoing inguinal hernia repair. Methods 70 cases of elderly patients with inguinal hernia treated by tension-free hernioplasty were randomly divided into local anesthesia group and continuous epidural anesthesia group with 35 cases in each group. Tension free repair of inguinal hernia was performed under local anesthesia and continuous epidural anesthesia, respectively. The levels of WBC, lymphocyte absolute value, interleukin-6 (IL-6) and tumor necrosis factor 伪 (TNF- 伪) were observed in the two groups at 1 day before anesthesia, 1 day after anesthesia, 1 day after operation and 3 days after operation. Results the level of WBC in local anesthesia group was significantly lower than that in continuous anesthesia group at T1 time point. The level of IL-6 in the epidural anesthesia group was significantly higher than that in the continuous epidural anesthesia group at the time point P0.05 / T 2. Conclusion the main immunological effects of local anesthesia on inguinal hernia repair in the elderly patients are significant higher than that in the continuous epidural anesthesia group. The influence of function index is small, It can be used as the first choice for tension-free repair of inguinal hernia in the elderly.
