环氧乙烷消毒麻醉机对COPD吸入麻醉患者气道炎症反应的影响

【摘要】:目的:探索不同麻醉机消毒方法对手术合并COPD吸入性麻醉患者气道炎症反应的影响。方法:有慢性阻塞性肺病(COPD)患者70例,随机均分为两组(n=35),观察组患者采用术前经环氧乙烷浸泡消毒的麻醉机进行麻醉;对照组患者则术前采用2%戊二醛溶液浸泡消毒麻醉机进行麻醉。检测两组插管后及插管后2h、4h血清及肺泡灌洗液中的TNF-α水平、IL-8、IL-2及IL-1β的浓度。结果:两组各血清因子浓度随时间延长而增加,插管后2h、4h各个指标浓度较刚插管后均显著增加(P均0.05)。两组肺泡灌洗液中各指标浓度随时间延长而增加,插管后2h、4h各个指标浓度较刚插管后均显著增加(P0.05)。IL-2浓度在各个时段观察组的浓度均显著低于对照组(P0.05)。TNF-α水平在插管后2h、4h时观察组浓度明显低于对照组,IL-8插管后2h时观察组浓度明显低于对照组,差异均有统计学意义(P0.05)。结论:COPD吸入性麻醉患者使用环氧乙烷消毒的麻醉机对气道炎症反应影响较小,环氧乙烷的消毒效果优于2%戊二醛溶液,对气道有一定的保护作用。
[Abstract]:Objective: to explore the effect of different anesthetic machine disinfection methods on airway inflammation in patients with COPD inhalation anesthesia. Methods: 70 patients with chronic obstructive pulmonary disease (COPD) were enrolled in this study. All the patients were randomly divided into two groups. The patients in the observation group were anesthetized with anaesthesia machine which was soaked and disinfected by ethylene oxide before operation. The patients in the control group were anesthetized with 2% glutaraldehyde solution immersion disinfectant anaesthesia machine before operation. The levels of TNF- 伪 and IL-8 IL-2 and IL-1 尾 in serum and alveolar lavage fluid were measured after intubation and 2 h after intubation. Results: each serum factor in both groups was determined. The concentration increases with time, Two hours after intubation, the concentration of each index was significantly higher than that after intubation (P < 0.05). The concentration of each index in alveolar lavage fluid of two groups increased with the prolongation of time. Two hours after intubation, the concentration of each index was significantly higher than that after intubation. The concentration of IL-2 in observation group was significantly lower than that in control group 2 hours after intubation. The concentration of TNF- 伪 in observation group was significantly lower than that in control group at 2 h after intubation and 2 h after intubation. The concentration of IL-2 in observation group was significantly lower than that in control group at 2 h after intubation. The concentration of observation group was significantly lower than that of control group. Conclusion the anesthetic machine disinfected by ethylene oxide has little effect on airway inflammation, and the disinfection effect of ethylene oxide is better than that of 2% glutaraldehyde solution, and it has a certain protective effect on airway.
