右旋美托咪定对胸腰椎骨折全身麻醉患者围拔管期血流动力学的影响

【摘要】:目的:对胸腰椎骨折的患者给予右旋美托咪定全身麻醉,分析围拔管期血流动力学的情况。方法:将我院接受治疗的74例胸腰椎骨折的患者平均分为两组进行麻醉,研究组患者在10min内静脉泵注负荷量1μg/kg的右旋美托咪定进行全麻,同时给予0.2~0.6μg/kg(kg·h)的速度进行维持,对照组给予常规诱导麻醉,术后拔管后停右旋美托咪定,对右旋美托咪定麻醉诱导、气管插管、拔管时的血流动力学情况和心脑血管并发症的情况进行分析。结果:两组患者在插管和拔管时的MAP、SpO2、HR均有明显的升高,且对照组的升高程度明显高于研究组,差异有统计学意义(P0.05),患者术后并发症的发生率组间数据对比差异无统计学意义(P0.05)。结论:采用右旋美托咪定进行全麻在围拔管时期具有较好的效果,可以有效控制患者的血流动力学变化,减少对患者心脏和血压的影响,有效减少拔管期的应激反应,维持血流动力学稳定,值得临床推广。
[Abstract]:Objective: to analyze hemodynamics in patients with thoracolumbar fracture treated with dexmetomidine during extubation. Methods: 74 patients with thoracolumbar fractures received treatment in our hospital were divided into two groups. Patients in the study group received general anesthesia with intravenous infusion of 1 渭 g / kg dexmetomidine within 10 minutes, and were maintained at the speed of 0.2V 0.6 渭 g / kg / kg 路h. The control group was given routine induction anesthesia, and after extubation, dexmetoimidine was stopped after extubation. The hemodynamics and cardiovascular and cerebrovascular complications during anesthesia induction, tracheal intubation, extubation of dexmetidine were analyzed. Results: MAPMA-SpO2 HR was significantly increased in both groups during intubation and extubation. The degree of increase in the control group was significantly higher than that in the study group. The difference was statistically significant (P 0.05), and there was no significant difference in the incidence of postoperative complications among the groups. Conclusion: the general anesthesia with dexmetomidine has a good effect in the period of extubation. It can effectively control the hemodynamic changes of patients, reduce the influence on heart and blood pressure, reduce the stress response during extubation, and maintain the hemodynamic stability, which is worthy of clinical promotion.
