耳穴埋籽实施超前镇痛干预混合痔患者术后疼痛的随机对照临床研究

【摘要】：目的：通过观察评价术前耳穴埋籽超前镇痛干预混合痔术后疼痛的临床疗效及安全性。为临床上寻找一种安全、有效、便捷、价廉,易被患者接纳的无创、无痛、无毒副作用的防治痔术后疼痛的方法。方法：选择2012年8月-2013年10月在成都中医药大学附属医院肛肠科住院符合纳入标准的60例混合痔患者,随机分为治疗组A(30例)和对照组B(30例)两组。两组均采取在腰俞穴麻醉下行外剥内扎术,治疗组A在术前采用耳穴埋籽的方法,对照组B则采用术后口服双氯芬酸钠双释放肠溶胶囊的方法。在术后2、10、24、48、72小时分别收集两组患者术后疼痛变化的积分及镇痛药物的使用情况,疼痛积分采用视觉模拟评分法(VAS)进行评分。最后,分析比较两组患者术后疼痛的情况和药物使用情况。结果：入选的60例混合痔患者,在年龄、性别方面无明显差异,具有可比性。1、分别对两组患者术后第2、10、24、48、72小时的疼痛分数进行组间比较,经成组t检验进行统计学分析均具有统计学意义(P0.05),治疗组A患者的VAS均值明显低于对照组B。2、治疗组A使用镇痛药物明显低于对照组B。3、两组均未发生严重不良反应。结论：术前给予耳穴埋籽干预术后疼痛和术后口服双氯芬酸钠双释放肠溶胶囊治疗混合痔术后疼痛均有疗效,但术前给予耳穴埋籽与术后按创口疼痛情况口服双氯芬酸钠双释放肠溶胶囊相比较有明显减少使用镇痛药物的优势。此法操作简便、安全、无明显毒副作用,在临床上值得选择应用和推广。
[Abstract]:Objective: to observe and evaluate the clinical efficacy and safety of preemptive analgesia at auricular acupoint burying seeds in treating postoperative pain of mixed hemorrhoids in order to find a safe, effective, convenient, cheap and easy to be accepted by patients. Methods of preventing and treating postoperative pain of hemorrhoids without toxic side effects. Methods: from August 2012 to October 2013, 60 patients with mixed hemorrhoids who were hospitalized in the Department of anal and intestine, affiliated Hospital of Chengdu University of traditional Chinese Medicine, were selected. Two groups were randomly divided into two groups: treatment group (30 cases) and control group (30 cases). Both groups were treated with external exfoliation and internal ligation under Yaoshu anesthesia, and group A were treated with the method of embedding seeds at auricular point before operation. The control group B was treated with oral diclofenac sodium double release enteric-coated capsule after operation. The scores of postoperative pain changes and the use of analgesic drugs were collected in two groups at 2: 10 and 24 4872 hours after operation. The pain score was evaluated by visual analogue score (VAS). Finally, the postoperative pain and drug use were analyzed and compared between the two groups. Results: there was no significant difference in age and sex between 60 patients with mixed hemorrhoids. The VAS of patients in treatment group A was significantly lower than that of control group B.2.The analgesic drug used in treatment group A was significantly lower than that in control group B.3. there were no serious adverse reactions in both groups. Conclusion: it is effective to treat postoperative pain of mixed hemorrhoids with auricular acupoint embedding seeds before operation and oral diclofenac sodium double release enteric-coated capsules after operation. However, compared with oral administration of diclofenac sodium double release enteric-soluble capsule according to the condition of wound pain after operation, preoperation of auricular acupoint burying seeds has the advantage of reducing the use of analgesic drugs. This method is simple, safe and has no obvious side effects. It is worth choosing and popularizing in clinic.
