灌注液下和黏弹剂下行白内障超声乳化吸除术后眼表差异性研究

【摘要】：目的对比观察多孔前房维持器连接灌注液下与黏弹剂下行白内障超声乳化吸除联合人工晶状体植入手术(PHACO+IOL植入术)后对眼表影响。方法纳入分析的年龄相关性白内障患者68例(68眼)随机分成两组。A组34例全程使用多孔前房维持器(anterior chamber maintainer,ACM)连接灌注液维持前房下行PHACO+IOL植入术,B组34例常规黏弹剂维持前房下行PHACO+IOL植入术。观察术前及术后1天、术后7天、术后30天的主观干燥异物感、泪膜破裂时间(BUT)以及角膜荧光染色并积分(CFS)及无表面麻醉下泪液分泌实验(SⅠt)的改变。结果1.两组内患者各项指标与术前相比,其中BUT在术后1天、7天、30天均较术前明显缩短,CFS在术后1天、7天高于术前,SⅠt在术后1天明显增加,术后7天降低。以上差异均具有统计学意义(P0.05)。2.A组术后1天的角膜荧光素染色并积分值改变量(3.47±0.71)比B组(2.94±0.78)多,A组术后1天的泪液分泌试验(SchirmerⅠ)改变量(4.50±1.60mm/5min)比B组(3.65±1.18 mm/5min)多,以上差异具有统计学意义(P0.05)。而在术后7天和术后30天各项观察指标的改变量,两组之间差异均无统计学意义(P0.05)。结论与常规白内障超声乳化手术比较,灌注液下行白内障超声乳化吸除在术后早期会增加泪液分泌量,轻微加重角膜上皮损伤,但术后中晚期并未加重眼表组织的损害。
