超声定位下髂腹股沟及髂腹下神经阻滞在小儿隐睾手术中的应用

【摘要】：目的观察超声定位下髂腹股沟及髂腹下神经阻滞在小儿隐睾手术中的临床效果。方法选取择期行隐睾手术患儿60例,随机分为髂腹股沟及髂腹下神经阻滞复合静脉全身麻醉(简称全麻)组(A组)和骶管阻滞复合静脉全麻组(B组)。记录入手术室时(T_0)、手术切皮时(T_1)、手术结束时(T_2)各时间点患儿的心率、血氧饱和度(SpO_2)和平均动脉压(MAP)的变化;记录麻醉起效时间、苏醒时间、术中芬太尼、丙泊酚追加次数及术中、术后患儿不良事件发生情况;比较两组患儿苏醒时(T_3)、术后1h(T_4)和术后2h(T_5)时的疼痛行为评估量表(FLACC)评分。结果两组患儿SpO_2和MAP在各时间点比较,差异无统计学意义(P0.05);A组麻醉起效时间明显短于B组(P0.05);两组术中及术后不良反应发生率低且差异无统计学意义(P0.05),两组苏醒时间及术后镇痛效果比较差异无统计学意义(P0.05)。结论超声定位下髂腹股沟及髂腹下神经阻滞应用于小儿隐睾手术麻醉效果安全有效,且方法简单、易于实施、起效较快,有利于患儿术中管理。
[Abstract]:Objective To observe the clinical effect of ilio inguinal and ilio hypogastric nerve block under ultrasound localization in children's cryptorchidism operation. Methods select 60 children with cryptorchidism, randomly divided into the ilio groin and ilioferastric nerve block combined general anesthesia (general anesthesia) group (group A) and the sacral block compound intravenous general anesthesia group (group B). The rate of heart rate, oxygen saturation (SpO_2) and mean arterial pressure (MAP) of the children at the time of operation (T_2) at the time of operation (T_0), the time of oxygen saturation (SpO_2) and the mean arterial pressure (MAP) at the end of the operation; the time of onset of anesthesia, the time of awakening, the number of fentanyl, the number of propofol in the operation, the incidence of adverse events in the operation, and the incidence of adverse events in the children after the operation; and the comparison between the two groups of children (T_3). The pain behavior assessment scale (FLACC) score of post 1H (T_4) and postoperative 2H (T_5). Results the difference of SpO_2 and MAP in the two groups was not statistically significant (P0.05), and the onset time of the A group was significantly shorter than the B group (P0.05); the incidence of adverse reactions in the two groups was low and the difference was not statistically significant (P0.05), and the two groups of awakening time. There is no significant difference in the effect of postoperative analgesia (P0.05). Conclusion the anesthesia effect of ilio inguinal and ilio hypogastric nerve block under ultrasonic localization in the operation of cryptorchidism in children is safe and effective, and the method is simple, easy to implement, rapid and beneficial to the management of children's operation.
