脊髓p38MAPK活化在瑞芬太尼诱发术后痛觉过敏中的作用

【摘要】:目的蛛网膜下腔注射p38MAPK抑制剂SB203580,观察其对瑞芬太尼输注后切开痛痛觉过敏的影响,及脊髓背角p-p38MAPK阳性细胞数目的变化,探讨脊髓背角p38MAPK活化在瑞芬太尼诱发术后痛觉过敏中的作用。方法取蛛网膜下腔置管成功的雄性SD大鼠40只,体重230-270g,采用数字表法将其随机分为5组:对照组、切口痛组、瑞芬太尼组、p38MAPK特异性抑制剂SB203580组(p38MAPK抑制剂组)和SB203580的溶媒二甲基亚砜DMSO组(溶媒二甲基亚砜组)。对照组、切口痛组、瑞芬太尼组大鼠蛛网膜下腔分别给予NS10μL,p38抑制剂组和溶媒二甲亚枫组大鼠蛛网膜下腔分别给予0.1%SB20358010μL和2%DMSO10μL;切口痛组大鼠在1.5%异氟烷吸入麻醉下行右侧跖底切口手术,对照组大鼠不行右侧跖底手术,其余三组均在瑞芬太尼持续泵注下行右侧跖底切开手术。采用热辐射刺激仪和von Frey纤维丝分别测定双侧后爪热刺激缩爪潜伏期(PWL)及机械性痛阈(PWT)。应用免疫组织化学法检测脊髓背角p-p38MAPK表达的变化。结果与切口痛组比较,瑞芬太尼组、溶媒二甲亚枫组在术后4h、1d、2d、3d双侧跖底PWL、PWT值均降低,双侧脊髓背角p-p38MAPK表达增多;与瑞芬太尼组比较,,p38抑制剂组双侧跖底的PWT值、手术侧跖底PWL值增高,双侧脊髓背角p-p38MAPK表达减少。结论脊髓背角p38MAPK活化可能参与了瑞芬太尼诱发的术后痛觉过敏。
[Abstract]:Objective to observe the effect of subarachnoid injection of p38MAPK inhibitor SB203580 on remifentanil after infusion of remifentanil and the changes of p-p38MAPK positive cells in the dorsal horn of spinal cord. To investigate the role of p38MAPK activation in remifentanil induced postoperative hyperalgesia. Methods Forty male Sprague-Dawley rats (230-270g) with successful subarachnoid catheterization were randomly divided into five groups: control group, incision pain group, and incision pain group. In remifentanil group, p38 MAPK specific inhibitor, SB203580 group, and SB203580 solute dimethyl sulfoxide DMSO group (dimethyl sulfoxide group). Control group, incision pain group, The rats in remifentanil group were given subarachnoid NS10 渭 L and 2%DMSO10 渭 L, respectively, and the rats in incision pain group were treated with right plantar floor incision under 1.5% isoflurane inhalation anesthesia, and the rats in remifentanil group were given 0.1%SB20358010 渭 L and 2%DMSO10 渭 L in subarachnoid space, respectively. Rats in the control group were not operated on the right plantar floor, and the other three groups were treated with remifentanil continuous pump for right plantar floor incision. Thermal radiation stimulator and von Frey filament were used to measure the latent period of paw contraction and mechanical pain threshold. Immunohistochemical method was used to detect the expression of p-p38MAPK in spinal dorsal horn. Results compared with incision pain group, remifentanil group and dimethyl submaple group decreased the PWT value of bilateral plantar floor PWL and increased the expression of p-p38MAPK in bilateral dorsal horn of spinal cord on the 1st and 2nd day after operation, and compared with remifentanil group, the PWT value of bilateral plantar floor in remifentanil group was higher than that in remifentanil group. The PWL of the plantar floor increased and the expression of p-p38MAPK decreased in the bilateral dorsal horn of spinal cord. Conclusion the activation of p38MAPK in spinal dorsal horn may be involved in postoperative hyperalgesia induced by remifentanil.
