麻醉困难气道处理模拟培训体系的构建与评价

【摘要】:目的: 利用先进的医学模拟设备结合最新的困难气道处理指南以及相关专业知识从临床实践出发建立科学的麻醉困难气道处理模拟培训体系,有针对性地提高受训者的实践操作能力,临床思维能力和自主学习能力,满足医院对从业者业务能力方面的需求,提高麻醉专业的学科地位和影响力以及病人安全性。 方法: 1.建立麻醉困难气道处理模拟培训体系 以麻醉专业知识内容为基础、依托于先进的医学模拟设备,结合最新的困难气道处理指南,建立麻醉困难气道处理模拟培训体系的框架结构,以及后续实践麻醉困难气道处理模拟培训体系的相关考核评价方法。 2.通过实践评价困难气道处理模拟培训体系的有效性和可行性 选择60名麻醉住院医师,训练前在高级生理驱动模拟人上模拟操作四个病例,记录处理困难气道的初次成绩,再对这60名住院医师进行困难气道处理模拟培训体系的培训,再使用核心内容相同的四个模拟病例检验麻醉住院医师的困难气道处理能力。将两者结果进行统计分析对比。 结果:建立困难气道处理模拟培训体系,经该体系培训后的麻醉医师模拟病例的通过率显著高于培训前通过率(P0.05),提示建立的困难气道处理模拟培训体系能够满足困难气道处理的临床培训需求。 结论:建立的麻醉困难气道处理模拟培训体系满足临床能够提高麻醉从业人员的困难气道处理能力以及临床业务能力的需求。
[Abstract]:Objective: to establish a scientific training system for anaesthesia difficult airway management by using advanced medical simulation equipment combined with the latest guidance of difficult airway management and related professional knowledge from clinical practice. To improve the trainees' practical ability, clinical thinking ability and autonomous learning ability, to meet the needs of hospital practitioners' professional ability, to improve the subject status and influence of anesthetic specialty and patient safety. The establishment of anaesthesia difficult airway management simulation training system based on anesthetic professional knowledge, relying on advanced medical simulation equipment, combined with the latest difficult airway management guidelines, The frame structure of anaesthesia difficult airway treatment simulation training system was established, and the related assessment methods of anaesthesia difficult airway treatment simulation training system were established. 2. Through practical evaluation of the effectiveness and feasibility of the training system for difficult airway management, 60 anesthetized residents were selected. Four cases were simulated and operated on an advanced physiologically driven simulator before training, and the initial scores of difficult airway management were recorded. The 60 residents were trained in the difficult airway management simulation training system, and four simulated cases with the same core content were used to test the difficult airway processing ability of the anesthetized residents. Results: the simulation training system of difficult airway processing was established. The passing rate of anaesthetist simulated cases after training was significantly higher than that of pre-training (P0.05), which suggested that the simulated training system for difficult airway management could meet the clinical training needs of difficult airway management. The anaesthesia difficult airway management simulation training system can meet the needs of clinical staff to improve the ability of difficult airway management and clinical skills.
