简便气道管理流程在快诱导气管插管应用价值的探讨

【摘要】：目的探讨简便气道管理流程在全身麻醉快诱导气管插管中的应用价值。 方法回顾分析2005年1月～2012年12月9251例全身麻醉快诱导气管插管病历资料。根据使用简便气道管理流程的时间将数据分为两组，，2009年1月～2012年12月使用简便气道流程期间的病历为A组（n=4214），2005年1月～2008年12月未使用简便气道流程期间的病历为B组（n=5037）。分析A组气道管理方案的构成比，用Kappa检验分析初步方案的应用效果，用χ2检验比较A组与B组声门显露Ⅲ～Ⅳ级气管插管方法的构成比。 结果1.A组4214例病历全部气管插管成功，未发生因气管插管导致的严重缺氧、脑损伤及心搏骤停。麻醉诱导期10.87%（458/4214）出现了气道问题。A组有3.70%（156/4214）出现了面罩通气问题：双手扣面罩通气占8.98%（14/156）、口咽通气道辅助面罩通气占89.74%（140/156）、使用喉罩通气占1.28%（2/156）。A组有7.17%（302/4214）出现了气管插管问题：经口盲探法气管插管占33.44%（102/302）、弹性橡胶探条辅助Macintosh喉镜气管插管占62.26%（188/302）、McCoy喉镜气管插管占3.97%（12/302）、纤维支气管镜气管插管占0.33%（1/302）。2.A组出现面罩通气问题后首选口咽通气道的Kappa值=0.95，出现气管插管问题后首选弹性橡胶探条的Kappa值=0.73。3.声门显露Ⅲ～Ⅳ级A组6.12%（258/4214），B组6.83%（344/5037），两组间比较无统计学差异(P＞0.05)。两组声门显露Ⅲ～Ⅳ级气管插管的方法比较有统计学差异(P＜0.01)： A组使用弹性橡胶探条辅助Macintosh喉镜气管插管的使用率为56.20%（145/258）、B组为40.40%（139/344）、两组间比较有统计学差异(P＜0.05)；A组经口盲探法39.15%（101/258）、B组50.58%（174/344）、两组间比较有统计学差异(P＜0.05)；A组其它方法的使用率4.65%(12/258)、B组9.01%（31/344）、两组间比较有统计学差异(P＜0.05)。 结论本简便气道管理流程具有一定的临床应用价值。
[Abstract]:Objective to investigate the application value of simple airway management procedure in fast induction tracheal intubation under general anesthesia. Methods from January 2005 to December 2012, 9251 cases of rapid induction tracheal intubation under general anesthesia were retrospectively analyzed. The data were divided into two groups according to the time of using the simple airway management process. The medical records for the period from January 2009 to December 2012 were group A (n = 4214). The medical records of group B during the period from January 2005 to December 2008 without the use of simple airway procedures were group B (n = 5037). The composition ratio of airway management scheme in group A was analyzed, the application effect of the preliminary scheme was analyzed by Kappa test, and the composition ratio of glottis exposure grade 鈪
