采用高胰岛素—正葡萄糖钳夹技术观察耳针对家兔胰岛素敏感性的影响

【摘要】:研究目的: 采用高胰岛素-正葡萄糖钳夹技术评价耳针对家兔血糖的干预作用,从而确定耳针是提高胰岛素敏感性的一种方法,并阐明其干预作用的机制;为耳针治疗2型糖尿病(T2DM)提供实验依据,为临床推广应用打下基础。 实验方法: 1.选取健康新西兰家兔12只,随机分为清醒组和麻醉组,每组6只(n=6)。清醒组:在非麻醉状态下,行家兔双侧耳缘静脉穿刺术。麻醉组:在麻醉状态下行家兔颈动静脉插管术。两组家兔分别进行高胰岛素-正葡萄糖钳夹实验,根据实验周期、血糖波动等因素对两种方法进行比较。 2.选取健康新西兰家兔16只,随机分为对照组和耳针组,每组8只(n=8)。耳针组:模拟人体取穴,在相当于人体耳穴胰点的家兔耳甲艇处,双侧,电针治疗,疏密波,40min。对照组:除未电针外,与耳针组相同。两组家兔分别采用高胰岛素-正葡萄糖钳夹技术检测葡萄糖输注率(GIR),并测定家兔空腹血糖(FBG)、钳夹实验期间血糖及稳态血糖,对家兔的胰岛素敏感性进行评估。实验结果: 1.非麻醉状态下的家兔双侧耳缘静脉穿刺的高胰岛素-正葡萄糖钳夹实验中家兔血糖在正常范围内波动,操作过程简单,成功率高;而麻醉状态下的家兔颈动静脉插管高胰岛素-正葡萄糖钳夹实验中:家兔血糖波动较大,且不在正常范围内,操作过程复杂,成功率低。 2.非麻醉状态下的家兔双侧耳缘静脉穿刺的高胰岛素-正葡萄糖钳夹实验中对照组和耳针组的GIR变异系数(CVGIR)分别为1.28%、0.21%,耳针组明显低于对照组(P0.05)。同时对照组、耳针组的空腹血糖与稳态血糖比较后差异无统计学意义(P0.05)。 3.非麻醉状态下的家兔双侧耳缘静脉穿刺的高胰岛素-正葡萄糖钳夹实验中:耳针组家兔的GIR明显升高,与对照组比较差异显著(P0.05)。耳针组与对照组家兔的GIR在各个时间点的比较:输糖后40min、50min及60min时,耳针组GIR升高,差异显著(P0.05)。 实验结论: 1.在非麻醉状态下,采用家兔双侧耳缘静脉穿刺的高胰岛素-正葡萄糖钳夹实验,具有一定的可行性和可靠性,稳定性高、操作简便、死亡率低、重复性好。即在经典方法的基础上,成功建立家兔高胰岛素-正葡萄糖钳夹模型,突破了国内基础研究中只在小动物(大鼠及小鼠)应用的限制。 2.实验过程中家兔血糖逐渐趋于稳定,钳夹实验的稳定性较好。而耳针组钳夹实验结果具有更高的稳定性。 3.在输入葡萄糖后的40min、50min、60min时耳针组与对照组有明显差异,提示耳针可有效提高机体的胰岛素敏感性,增加外周组织对糖的利用率。它可能是通过改善肝脏和肌肉脂肪等外周组织对胰岛素的敏感性,提高周围组织对葡萄糖的利用率这一途径来调节机体的糖代谢,最终达到降低血糖的效果。
[Abstract]:Objective: to evaluate the effect of hyperinsulin-glucose clamp on blood glucose in rabbits, and to determine that ear acupuncture is a method to increase insulin sensitivity and clarify the mechanism of intervention. To provide experimental basis for auricular acupuncture treatment of type 2 diabetes mellitus (T2DM) and lay a foundation for clinical application. Experimental method: 1. Twelve healthy New Zealand rabbits were randomly divided into awake group and anesthetic group with 6 rabbits in each group. Lucid group: under the condition of non-anaesthesia, bilateral auricular vein puncture was performed in rabbits. Anesthesia group: cervical arteriovenous intubation was performed in anesthetized rabbits. The hyperinsulin-glucose clamp test was carried out in two groups of rabbits, and the two methods were compared according to the experimental period and the fluctuation of blood glucose. 2. Sixteen healthy New Zealand rabbits were randomly divided into control group and ear acupuncture group (n = 8). Auricular acupuncture group: simulated human body to take points, at the rabbit ear nail boat equivalent to the human ear point pancreatic point, bilateral, electroacupuncture treatment, dredge wave, 40 minutes. The control group was the same as the ear acupuncture group except for no electroacupuncture. Hyperinsulin-positive glucose clamp technique was used to detect glucose infusion rate (GIR),) and fasting blood glucose (FBG),) clamp in two groups. The insulin sensitivity of rabbits was evaluated. Experimental results: 1. In the hyperinsulin-positive glucose clamp experiment of bilateral auricular vein puncture in non-anesthetized rabbits, the blood glucose fluctuated within the normal range, the operation process was simple and the success rate was high. However, in the hyperinsulin-positive glucose clamp experiment of rabbit cervical arteriovenous intubation under anesthesia, the blood glucose fluctuated greatly and was not within the normal range, the operation process was complicated and the success rate was low. 2. In the hyperinsulin-positive glucose clamp test of bilateral auricular vein puncture in non-anesthetized rabbits, the (CVGIR) of GIR in the control group and the ear acupuncture group was 1.28 and 0.21, respectively, and that in the ear acupuncture group was significantly lower than that in the control group (P0.05). At the same time, there was no significant difference in fasting blood glucose and steady-state blood glucose between the control group and the ear acupuncture group (P0.05). 3. In the hyperinsulin-positive glucose clamp experiment of bilateral auricular margin vein puncture in non-anesthetized rabbits, the GIR of the auricular acupuncture group was significantly higher than that of the control group (P0.05). The comparison of GIR between the auricular acupuncture group and the control group at each time point: the GIR of the auricular acupuncture group was significantly higher than that of the control group (P0.05). Experimental conclusions: 1. Under the condition of non-anaesthesia, the high insulin-glucose clamp test with double lateral auricular vein puncture in rabbits is feasible and reliable, with high stability, simple operation, low mortality and good reproducibility. On the basis of the classical method, the rabbit hyperinsulin-glucose clamp model was successfully established, which broke through the limitation of application in domestic basic research only in small animals (rats and mice). 2. During the experiment, the blood sugar of rabbits tended to be stable, and the stability of clamp experiment was better. The results of the pinching experiment in the ear acupuncture group were more stable than those in the ear acupuncture group. 3. There was a significant difference between the auricular acupuncture group and the control group at 40 min, 50 min and 60 min after glucose infusion, which suggested that ear acupuncture could effectively improve the insulin sensitivity of the body and increase the utilization rate of glucose in peripheral tissues. It may regulate glucose metabolism by improving the sensitivity of peripheral tissues such as liver and muscle fat to insulin and increasing the utilization rate of glucose in peripheral tissues.
