星状神经节阻滞对SD大鼠离体心功能的影响

【摘要】:目的:SD大鼠行星状神经节阻滞,观察星状神经节阻滞对Langendorff离体心脏灌流装置离体心功能的影响,为星状神经节阻滞用于临床改善心功能减少围手术期心血管事件发生提供理论依据。 方法:将24只8~10周龄体重180~220克的雄性SD大鼠采用随机数字表法随机分为3组,每组8只:(1)左侧星状神经节阻滞组(LS组);(2)右侧星状神经节阻滞组(RS组);(3)手术对照组(C组)。大鼠腹腔内注射完成麻醉后,钝性分开左右胸头肌,沿食管旁找到星状神经节,埋管并缝皮,7天后行星状神经节阻滞实验,观察是否出现霍纳综合征,并连续给药两周。星状神经节阻滞模型建立成功后,应用Langendorff离体心脏灌流装置建立SD大鼠离体心脏灌流模型。大鼠腹腔注射肝素后,仰卧位固定,迅速开胸,取出心脏,置于4℃冷的K-H液中分离主动脉,悬挂于Langendorff离体心脏灌流装置上。K-H液灌注15~20分钟,达到平衡末时测定冠脉流量(CF),记录相关的心功能指标:心率(HR)左室发展压(LVDP)左室舒张末压(LVEDP)收缩期心室内压力升高速率(+dp/dt)舒张期心室内压力升高速率(-dp/dt)。同时取下心脏离心后取上清,经过加标准品加样加抗体洗涤以及加底物等步骤后,用ELISA法检测心肌组织中肾上腺素(EPI)去甲肾上腺素(NE)神经肽Y(NPY)和血管活性肠肽(VIP)的浓度。 结果:1.与C组相比,RSGB组LVDP+dp/dtmax-dp/dtmaxCF及HR值均升高有统计学意义(P<0.05或0.01),RSGB组的LVEDP值降低有统计学意义(P<0.01);与C组相比,LSGB组LVDP+dp/dtmax-dp/dtmax及CF值均升高有统计学意义(P<0.05或0.01),LSGB组的LVEDP值降低有统计学意义(P<0.05)。与LSGB组相比,RSGB组LVDP+dp/dtmax及-dp/dtmax值升高有统计学意义(P<0.05),RSGB组LVEDP值降低有统计学意义(P<0.05)。2.与C组相比,RSGB组和LSGB组EPINENPY及VIP值均升高有统计学意义(P<0.05或0.01)。与LSGB组相比,RSGB组EPINE及NPY值均升高有统计学意义(P<0.05)。结论:1.星状神经节阻滞通过增强心肌的收缩和舒张功能,对SD大鼠离体心功能有促进作用;2.左右两侧星状神经节对心脏的交感神经支配区域不同,,两侧星状神经节阻滞对大鼠离体心功能的影响存在差异,右侧星状神经节阻滞增强心功能的效果更佳。
