双J管弹性的体外试验与临床初步研究

【摘要】:【目的】 通过对双J管弹性的体外试验与临床研究,进一步指导双J管在临床中的合理应用,从而减少双J管并发症的发生。 【方法】 1、体外实验:将同批号的双J管60根分别浸泡于蒸馏水组:(A组);酸性尿组:(B组);碱性尿组:(C组)。浸泡15天,30天,60天,120天后取出双J管测量其弹性、重量。 2、临床观察:将2012年12月至2013年12月在郴州市第一人民医院泌尿外科手术术后留置双J管病人100例。按留置体内时间分组:15~29天:D组(D组42例);30~59天:E组(E组48例);≥60天:F组(F组10例)。拔出双J管后将其滤干,称重量。再将双J管悬挂在自制弹性测量架上,测量伸展度(弹性)。 【结果】 1.分析体外实验中同组内双J管置于不同时间后再相同牵拉方向下有无弹性差异。对A、B、C组内分别进行两两比较t检验,在蒸馏水组(A组)中P值均0.05,不具有统计学意义。在酸性尿组(B组)与碱性尿组(C组)中,浸泡时间30天的组P值均0.05,不具有统计学意义;浸泡时间60天组P值均0.05,认为同一组内同型号的双J管短时间内(30天)其弹性无明显变化,长时间留置双J管(60天)其弹性越差。 2.分析体外实验中同组内双J管置于不同时间后有无重量变化差异。对A、B、C三组内分别进行两两比较t检验,在蒸馏水组(A组)中P值均0.05,不具有统计学意义。在酸性尿组(B组)与碱性尿组(C组)中,浸泡时间30天的组P值均0.05不具有统计学意义;浸泡时间60天组P值均0.05,认为同一组内同型号置于时间较久的双J管其重量较时间短的双J管重量要重。 3.分析体外实验中不同组间同种型号双J管弹性有无差异。即置于不同液体的双J管弹性有无差异。对A、B、C三组进行方差分析两两比较LSD-t检验、其中B组与C组P值0.05,不具有统计学意义,无显著性差异,认为双J管置于酸性尿与碱性尿中弹性无显著差异。A组与B组、A组与C组P值均0.05,具有统计学意义,认为双J管分别置于相同时间的尿液和蒸馏水中,前者弹性要差于后者。 4.分析体外实验中不同组间同种型号双J管重量变化有无差异。对A、B、C三组进行方差分析两两比较LSD-t检验、其中B组与C组P值0.05,不具有统计学意义,无显著性差异,认为双J管置于酸性尿与碱性尿中重量无显著差异。A组与B组、A组与C组P值均0.05,具有统计学意义,认为相同时间置于尿液中的双J管重量比置于纯净水中双J管要重。 5.分析临床观察中所收集的双J管进行不同组间D-J管弹性有无差异。即病人体内置管时间与双J管弹性有无关系。对D、E、F三组进行方差分析两两比较LSD-t检验,D组与E组P0.05不具有统计学意义。D组与F组,E组与F组P0.05具有统计学意义,可认为双J管在患者体内超过60天,其弹性可能会变差。 6.分析临床观察中不同组间双J管重量有无差异。对D、E、F三组进行方差分析两两比较LSD-t检验,D组与E组P0.05不具有统计学意义。D组与F组,E组与F组P0.05具有统计学意义,可认为双J管在患者体内时间超过60天,其重量明显变重,发生尿垢附着越明显。 【结论】 1.双J管置于蒸馏水中,120天内其弹性及重量变化未见明显改变。 2.浸泡时间相同时,,双J管分别置于酸性尿液(PH=6.5)及碱性尿液(PH=7.5)中弹性及重量无明显差异。 3.临床中短期留置双J管(30天)不会对其弹性及重量产生影响,反之长期留置(60天)其弹性可能会越差,自身重量越重。
[Abstract]:[Objective]
In vitro experiments and clinical studies of double J tube elasticity, we further guide the rational application of double J tube in clinical practice, thus reducing the incidence of complications of double J tube.
[method]
1, in vitro experiments: the same batch of double J tubes and 60 roots were soaked in distilled water respectively: group A (group B); acidic urine group: (group B); alkaline urine group: (group C). Soaked for 15 days, 30 days, 60 days, 120 days later, the double J tube was taken out to measure its elasticity and weight.
2. Clinical observation: from December 2012 to December 2013 in the Department of Urology of Chenzhou First People's Hospital after surgery of 100 cases of patients with indwelling double J tube. According to the retention time of packet: 15~29 days: D group (group D, 42 cases); 30~59 days: E group (group E, 48 cases); at least 60 days: F group (group F 10 cases). Pull out the double J tube after the drain and weighed. Then the double J tube hanging in the self made elastic measuring rack, measuring stretch (elastic).
[results]
1. analysis of the in vitro experiments within the same group of double J tube in different time after the same pulling direction without the elastic differences. For A, B, 22 t test respectively. In C group, the distilled water group (group A) in P was 0.05, was not statistically significant in acidic urine group. (group B) and alkaline urine group (C group), soaking time 30 days of group P were 0.05, not statistically significant; soaking time of 60 days group P value was 0.05, that of double J in the same group with the same type of pipe within a short period of time (30 days) the elastic had no obvious change long time, indwelling double J tube (60 days) the elasticity is poor.
2. analysis of the in vitro experiments within the same group of double J tube in different time after there is no weight difference. For A, B, 22 t test respectively. C in the three groups, the distilled water group (group A) in P was 0.05, was not statistically significant. In acidic urine group (B group) with alkaline urine group (C group), soaking time 30 days of group P were 0.05 was not statistically significant; soaking time of 60 days group P value was 0.05, that of double J in the same group with the same type in the long tube of its weight is short of double J tube to heavy weight.
3. in vitro analysis between different groups of the same type of double J tube elasticity has no difference. That is double J tube under different liquid elasticity. No difference of A, B, C of three groups were analyzed by analysis of variance 22 comparison of LSD-t test, the B group and C group, the P value is 0.05, not statistically significant, no significant difference, that double J tube in acidic urine and alkaline urine elastic no significant difference between.A group and B group, A group and C group, P value was 0.05, with statistical significance, the same time that the double J tubes were placed in the urine and distilled water, the former elastic worse than the latter.
4. in vitro analysis between different groups of the same type of double J tube weight changes. No difference of A, B, C of three groups were analyzed by analysis of variance 22 comparison of LSD-t test, the B group and C group, the P value is 0.05, not statistically significant, no significant difference, that double J tube weight in acidic urine and alkaline urine no significant difference between.A group and B group, A group and C group, P value was 0.05, with statistical significance, the same time that the double J placed in the urine tube weight to be heavier than in pure water and double J tube.
Double J collected 5. clinical observation and analysis of the tube of D-J among different groups have no difference. The elastic tube body disease time of the tube and double tube elastic J has no relationship. For D, E, F of three groups were analyzed by analysis of variance 22 comparison of LSD-t test, D group and E group P0.05 was not statistically significant.D group and F group, E group and F group P0.05 has statistical significance, can think of double J tube in patients more than 60 days, the elasticity may deteriorate.
6. clinical observation and analysis in different groups of double J tube weight. No difference of D, E, F of three groups were analyzed by analysis of variance 22 comparison of LSD-t test, D group and E group P0.05 was not statistically significant in.D group and F group, E group and F group P0.05 has statistical significance, can be considered in double J tube time in patients with more than 60 days, the weight was heavier, urinary scale attached more obvious.
[Conclusion]
1. double J tubes were placed in distilled water, and the change of their elasticity and weight did not change in 120 days.
2. the elasticity and weight of the double J tube were not significantly different in the acid urine (PH=6.5) and the alkaline urine (PH=7.5) when the soaking time was the same.
3. in clinical short term, double J tube (30 days) will not affect its elasticity and weight. On the contrary, the longer the long-term retention (60 days), the more its elasticity may be worse, the heavier the weight of its own.
