经后腹腔自制单孔通道与传统后腹腔镜治疗T1期肾癌的疗效比较

【摘要】：目的探讨经后腹腔自制单孔通道腹腔镜肾癌根治术治疗T1期肾癌的临床应用价值。方法 T1期肾癌患者80例,其中应用自制通道行后腹腔单孔腹腔镜肾癌根治术40例,传统后腹腔镜肾癌根治术40例;回顾性分析和比较两组患者在手术时间、术中出血量、术后住院时间、术后疼痛评分、切口美容满意度、手术并发症及生存率上的差异。结果两组手术均顺利完成,无中转开放或传统腹腔镜者。经后腹腔自制单孔通道腹腔镜组与传统后腹腔镜组的术后疼痛评分〔(3.63±0.87)分vs(4.52±1.24)分〕、切口美容满意度〔(7.95±0.60)分vs(6.93±1.21)分〕差异显著(P0.05)。术后随访时间3~40个月,两组患者均未出现肿瘤复发或转移。结论经后腹腔自制单孔通道腹腔镜肾癌根治术是安全可行的。与传统后腹腔镜肾癌根治术相比,术后疼痛更轻、切口美容满意度更高。肿瘤治疗的远期疗效仍需进一步长期随访予以验证。
[Abstract]:Objective to evaluate the clinical value of retroperitoneal single channel laparoscopic radical nephrectomy for T1 stage renal cell carcinoma. Methods A total of 80 patients with T1 stage renal cell carcinoma were treated with self-made channels, including 40 patients with retroperitoneal laparoscopic radical nephrectomy and 40 patients with traditional retroperitoneal laparoscopic renal cell carcinoma, the time of operation and the amount of intraoperative bleeding were retrospectively analyzed and compared between the two groups. Differences in postoperative hospital stay, postoperative pain score, incision beauty satisfaction, surgical complications and survival rate. Results the operation was completed successfully in both groups, and there was no transition to open operation or traditional laparoscopy. The postoperative pain score was 3.63 卤0.87 (vs(4.52 卤1.24) and the incision beauty satisfaction was 7.95 卤0.60 (vs(6.93 卤1.21). The follow-up time was 3 ~ 40 months. No tumor recurrence or metastasis was found in the two groups. Conclusion Laparoscopic radical resection of renal cell carcinoma with self-made self-made peritoneal cavity is safe and feasible. Compared with traditional retroperitoneal laparoscopic radical nephrectomy, postoperative pain is lighter and incision beauty satisfaction is higher. The long-term effect of tumor treatment needs further long-term follow-up to verify.
