医院非淋球菌性尿道炎患者支原体感染和耐药状况研究

【摘要】:目的探究医院非淋球菌性尿道炎患者支原体感染和耐药状况,以期临床治疗提供参考。方法选取2013年1月-2015年12月医院收治152例非淋球菌性尿道炎患者为研究对象,均进行支原体培养和药敏检测,并分析支原体对各类抗菌药物的耐药性变迁。结果 2013-2015年间支原体培养阳性率逐年升高,但各类支原体所占比例无显著变化,大部分为Uu,其次为Uu+Mh,Mh最少;2013年-2015年间不同支原体来源由高到低为宫颈分泌物、男性尿道分泌物;女性支原体感染率显著高于男性;支原体感染率最高的年龄段为21~30岁,其次为31~40岁,差异有统计学意义(P0.05);2013-2015年Uu+Mh对多西环素、米诺环素、克雷霉素、交沙霉素,Uu对环丙沙星的耐药率变化差异无统计学意义;2013-2015年Uu+Mh和Uu对多诺环素、环酯红霉素、阿奇霉素、克拉霉素、罗红霉素、红霉素、左氧氟沙星、司帕沙星、甲砜霉素等耐药性逐年增高,差异有统计学意义(P0.05)。结论支原体感染多发于年龄21~30岁女性,且发病率逐年增高,对各类常用抗菌药物的耐药性也逐年增高,多西环素、米诺环素、交沙霉素成为治疗的首选药物,临床中可依据支原体培养和药敏检测选择适当的抗菌药物进行治疗。
[Abstract]:Objective to investigate mycoplasma infection and drug resistance in patients with nongonococcal urethritis. Methods 152 patients with nongonococcal urethritis treated in our hospital from January 2013 to December 2015 were studied. Mycoplasma culture and antimicrobial susceptibility were detected, and the changes of drug resistance of mycoplasma to various antimicrobial agents were analyzed. Results the positive rate of mycoplasma culture increased year by year from 2013 to 2015, but the proportion of mycoplasma was not significantly changed, most of them were Uuu, followed by UU Mhmeng Mh. From 2013 to 2015, different mycoplasma sources from high to low were cervical secretions. The infection rate of urethral secretion in male was significantly higher than that in male. The highest rate of mycoplasma infection was in the age group of 21 ~ 30 years old, followed by 31 ~ 40 years old. The difference was statistically significant (P 0.05). The UU Mh of doxycycline, minocycline, clemycin from 2013 to 2015 was significantly higher than that of male. There was no significant difference in drug resistance of roxithromycin Uu to ciprofloxacin from 2013 to 2015. Uu Mh and Uu to donocycline, cycloester erythromycin, azithromycin, clarithromycin, roxithromycin, levofloxacin, sparfloxacin from 2013 to 2015. The drug resistance of sulfomycin and other drugs increased year by year, and the difference was statistically significant (P 0.05). Conclusion Mycoplasma infection mostly occurs in women aged 21 to 30 years, and the incidence of mycoplasma infection is increasing year by year, and drug resistance to various commonly used antimicrobial agents is also increasing year by year. Doxycycline, minocycline and josamycin are the first choice in the treatment of mycoplasma. Appropriate antimicrobial agents can be selected for treatment according to mycoplasma culture and drug sensitivity test.
