153例IgA肾病临床与病理特征分析

【摘要】：目的探讨IgA肾病患者临床表现、病理特点及其相关性。方法对我院收治的153例IgA肾病患者相关资料进行综合性回顾性分析,入选患者均进行过肾活检病理确诊为IgA肾病,分析IgA肾病临床表现、病理类型等。结果 40例患者为有诱因发病,其中22例上呼吸道感染、3例妊娠、7例疲劳、4例肠道感染,1例由于海鲜引起过敏,1例腰部扭伤,2例手术原因;Ι级病理患者中6例为蛋白尿联合血尿型,Ⅱ级病理中7例单纯血尿型,10例蛋白尿联合血尿型;Ⅲ级病理中,20例蛋白尿联合血尿型;Ⅳ级病理中27例蛋白尿联合血尿型;Ⅴ级病理中5例单纯血尿型,5例蛋白尿联合血尿型;免疫复合IgA中有12例,占7.8%,IgA+Ig M占13.7%;IgA+C3占17.0%;IgA+Ig G+Ig M占5.2%;IgA+Ig G+C3占7.2%;IgA+Ig M+C3占32.7%;IgA+Ig G+Ig M+C3占17%。结论 IgA肾病在感染诱因下发病率较高,患者发病后临床表现以蛋白尿联合血尿型为主;同时,肾衰竭型比例出现上升趋势,且患者治疗预后不佳,临床上应该根据患者病理结果进行积极有效治疗,降低肾衰竭发生率。
[Abstract]:Objective to investigate the clinical manifestations, pathological features and correlation of IgA nephropathy. Methods the data of 153 patients with IgA nephropathy in our hospital were analyzed retrospectively. All the patients were diagnosed as IgA nephropathy by renal biopsy, and the clinical manifestations and pathological types of IgA nephropathy were analyzed. Results there were 40 patients with predisposing disease. There were 22 cases of upper respiratory tract infection 3 cases of pregnancy, 7 cases of tiredness, 4 cases of intestinal infection, 1 case of seafood allergy, 1 case of lumbar sprain, 2 cases of surgical causes, 6 cases of proteinuria combined with hematuria, 6 cases of grade I pathology.
