蛋白尿与免疫球蛋白A肾病患者Lee氏分级的相关性及疾病影响因素分析

【摘要】:目的探讨蛋白尿与Lee氏分级与免疫球蛋白A肾病(IgAN)患者肾损伤程度的相关性分析。方法选取深圳市第二人民医院2010年12月—2016年10月登记的IgAN患者507例,根据Lee氏分级分为低级别病理组(n=163)和高级别病理组(n=344)。比较两组患者清蛋白(Alb)、24 h蛋白尿定量、血肌酐(Scr)、血尿酸(UA)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)及低密度脂蛋白胆固醇(LDL-C),并分析蛋白尿与Lee氏分级的相关性及其与免疫球蛋白A肾病患者肾损伤程度影响因素分析。结果高级别病理组患者Alb、24 h蛋白尿定量、Scr、UA、TC、TG、LDL-C高于低级别病理组,HDL-C低于低级别病理组(P0.05)。IgA肾病肾损伤程度多因素logistic回归分析结果显示,Lee氏分级、24 h蛋白尿定量是影响IgA肾病肾损伤程度的独立危险因素(P0.05)。肾脏病理Lee氏分级与24 h尿蛋白定量成正相关(P0.05)。结论蛋白尿与Lee氏分级呈正相关,且Lee氏分级、24 h蛋白尿定量是影响IgA肾病肾损伤程度的独立危险因素。
[Abstract]:Objective to investigate the correlation between proteinuria and Lee's grade and IgANs in patients with immunoglobulin A nephropathy. Methods 507 IgAN patients registered in Shenzhen second people's Hospital from December 2010 to October 2016 were divided into low grade pathological group (n = 163) and high grade pathology group (n = 3444) according to Lee's grade. 24 h proteinuria was compared between the two groups. Serum creatinine (SCR), serum uric acid (UAA), total cholesterol (TC), triglyceride (TGG), high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterol (LDL-C) were analyzed. The correlation between proteinuria and Lee's grade and renal injury in patients with immunoglobulin A nephropathy were analyzed. Analysis of influencing factors of degree. Results the results of logistic regression analysis showed that 24 h proteinuria in high grade pathological group was significantly lower than that in low grade pathological group (P 0.05). The results of multivariate logistic regression analysis showed that the 24 h proteinuria quantitative analysis of Lee's grade and 24 h proteinuria was related to IgA. The independent risk factor of renal injury was P0.05. There was a positive correlation between Lee's grade of renal pathology and 24 h urinary protein quantification (P0.05). Conclusion there is a positive correlation between proteinuria and Lee's grade, and the quantitative analysis of proteinuria in 24 hours is an independent risk factor for renal injury of IgA nephropathy.
