常规放射检查对人类精子影响的初步研究

【摘要】:研究目的:电离辐射对人类生殖系统的影响一直备受关注,已有的大量研究结果已证实大剂量的电离辐射对生殖系统有确定的损伤效应,但低剂量电离辐射的影响仍尚未明确。医疗照射被认为是公众所接受的最大人工电离辐射照射来源,其中以常规放射检查最为常见,但尚未发现有常规放射检查对人类生殖健康的影响研究的报道。另外,冷冻精液标本在长期储存以及运输过程中可能受到的电离辐射影响仍未明确。因此本研究希望通过观察常规放射检查对人类新鲜精液的精子活力、精子存活率、精子运动特征及精子DNA完整性等指标的影响,初步探讨男性受到低剂量X射线照射后的生殖安全性。 研究方法:21份新鲜精液标本来源于浙江省人类精子库的供精志愿者,禁欲3-5d,手淫取精,精液置37℃恒温箱液化后将每份精液分成3组(对照组、1mGy组、50mGy组),1mGy组、50mGy组的精液样本分别接受1mGy(相当于普通平片的剂量)和50mGy(相当于CT检查的剂量)的X线照射。x线照射后分别对3组精液样本的精子活力、精子存活率、精子运动特征及精子DNA损伤等指标进行检测,精子活力按《世界卫生组织人类精液检查与处理实验室手册》(第5版)的标准操作程序,制备湿片后在相差显微镜下进行评估;精子存活率使用伊红-苯胺黑染料拒染法检测,每份样本观察的精子数至少在200个以上;精子运动特征参数用计算机辅助精子分析(CASA)进行分析,每份样本至少要分析200个活动精子的运动轨迹;精子DNA完整性检测使用基于吖啶橙染色的精子核完整性染色试剂盒,通过流式细胞仪进行检测。通过配对t检验比较不同组别指标间的差异。 研究结果: 1、人类新鲜精液经不同低剂量X射线照射后精子活力没有受影响。50mGy组、1mGy组的精子活力(43.52%±27.61%、46.24%±25.24%)与对照组的精子活力(43.14%±28%)没有显著性差异,P0.05;50mGy组、1mGy组没有显著性差异,P0.05。 2、人类新鲜精液经不同低剂量X射线照射后精子存活率没有受影响。50mGy组、1mGy组的精子活力(84.95%±8.07%、84.33%±8.64%)与对照组的精子活力(84.61%±7.76%)没有显著性差异,P0.05;50mGy组、1mGy组没有显著性差异,P0.05。 3、人类新鲜精液经不同低剂量X射线照射后精子运动特征参数没有受影响。50mGy组、1mGy组以及对照组平均的直线速率(VCL)、曲线速率(VSL)、平均路线速率(VAP)、精子头侧摆幅度(ALH)、直线性(LIN)、前向行(STR)、鞭打频率(BCF)均没有统计学差异,P0.05。 4、人类新鲜精液经不同低剂量X射线照射后精子DNA碎片指数(DFI)没有受影响。50mGy组、1mGy组的DFI(15.75%±10.91%、16.29%±12.76%)与对照组的DFI(16.48%±13.37%)没有显著性差异,P0.05;50mGy组、1mGy组没有显著性差异,P0.05。 5、人类新鲜精液经不同低剂量X射线照射后精子高DNA可染性(HDS)没有受影响。50mGy组、1mGy组的HDS(9.26%±4.23%、9.14%±4.25%)与对照组的HDS(9.29%±3.59%)没有显著性差异,P0.05;50mGy组、1mGy组没有显著性差异,P0.05。 研究结论:我们的研究表明50mGy及以下的低剂量X射线对人类新鲜精液的精子活力、精子存活率、精子运动特征参数以及精子DNA完整性等指标没有影响,因此,我们认为常规的放射学检查(平片、CT)对男性离体成熟的精子是相对安全的,对评估冷冻精液在长途运输中接受X射线安检的影响具有参考意义。但有关低剂量X射线对男性整体生育功能是否有影响,还需进一步深入研究。
[Abstract]:The effects of ionizing radiation on the reproductive health of human beings have been paid attention to . The effects of high doses of ionizing radiation on reproductive health have not been clarified .
The sperm motility , sperm viability , sperm motility and sperm DNA damage were measured in 3 groups ( control group , 1 mGy group , 50 mGy group ) , 1 mGy group and 50 mGy group respectively .
Sperm viability was detected using the eosin - aniline black dye exclusion assay , and the number of sperm observed per sample was at least 200 ;
The sperm motility parameters were analyzed by computer - assisted sperm analysis ( CASA ) , at least 200 active sperm motility trajectories were analyzed for each sample , and sperm DNA integrity testing was performed using an acridine orange stain - based sperm nucleus integrity staining kit , which was detected by flow cytometry . Differences between different groups were compared by paired t tests .
Results of the study :
1 . There was no significant difference between the sperm motility ( 43.52 % 卤 27.61 % , 46.24 % 卤 25.24 % ) and the sperm motility ( 43.14 % 卤 28 % ) in the 50 mGy group and 1 mGy group . There was no significant difference in sperm motility ( 43.14 % 卤 28 % ) in the control group .
There was no significant difference in sperm motility ( 84.95 % 卤 8.07 % , 84.33 % 卤 8.64 % ) in 50 mGy group and 1 mGy group ( 84.33 % 卤 8.64 % ) and control group ( 84.61 % 卤 7.76 % ) .
3 . The parameters of sperm motility were not influenced by human fresh semen after X - ray irradiation at different doses . The average linear velocity ( VCL ) , curve rate ( VCL ) , average route rate ( vap ) , head - side swing amplitude ( ALH ) , linearity ( LIN ) , forward line ( STR ) , and whips frequency ( Bp ) in 50 mGy group , 1 mGy group and control group were not statistically significant , P0.05 .
4 . There was no significant difference in the sperm DNA fragment index of human fresh semen after low dose X - ray irradiation ( 15.75 % 卤 10.91 % , 16.29 % 卤 12.76 % ) in the 50mGy group and 1 mGy group compared with the control group ( 16.48 % 卤 13.37 % ) , P0.05 ;
There was no significant difference between the 50mGy group and the 1mGy group ( P0.05 ) .
5 . There was no significant difference between the HDS ( 9.26 % 卤 4.23 % , 9.14 % 卤 4.25 % ) and HDS ( 9.29 % 卤 3.59 % ) of the control group ( P0.05 ) .
There was no significant difference between the 50mGy group and the 1mGy group ( P0.05 ) .
Conclusion : Our study shows that 50 mGy and lower dose X - rays have no influence on sperm motility , sperm viability , sperm motility , sperm DNA integrity , and so on . Therefore , we believe that routine radiological examination ( plain film , CT ) is relatively safe for male ex vivo sperm . However , it is of reference to evaluate the effect of low - dose X - ray on the whole reproductive function of men .
