点触式探针在铸型结石超声碎石术中的应用探讨

【摘要】：目的探讨点触式探针在铸型结石超声碎石清石手术的安全性与可行性。方法将45例肾铸型结石随机分成两组,观察组22例,对照组23例。两组分别用点触式探针和原平面超声探针行经皮肾镜超声碎石清石术。术中观察两种探针对尿路黏膜损伤情况,并准确记录碎石时间和结石体积,计算碎石清石速度。结果点触式探针的碎石清石时间和速度明显优于平面探针(P0.05),其速度是原超声探针的1.9倍。对照组出现多例轻微的黏膜吸附伤,无需处理,观察组未出现尿路黏膜吸附伤和刺伤。观察组和对照组结石清石率分别为95.45%(21/22)和91.33%(21/23),两组差异无统计学意义。结论点触式探针碎石速度快,效率高,安全性好,有较好的临床推广应用价值。
[Abstract]:Objective to investigate the safety and feasibility of point-touch probe in ultrasonic lithotripsy for cast stone. Methods 45 cases of renal cast stones were randomly divided into two groups: observation group (n = 22) and control group (n = 23). Two groups were treated by percutaneous nephroscopic lithotripsy with point touch probe and original plane ultrasonic probe. Intraoperative observation of urinary tract mucosal injury with two probes, accurate recording of lithotripsy time and volume of stone, and calculation of lithotripsy clearance speed. Results the lithotripsy time and velocity of point touch probe were better than that of planar probe (P0.05), and the velocity was 1.9 times of that of the original ultrasonic probe. In the control group, there were several slight mucosal adhesion injuries without treatment, and no urinary mucosal adhesion injury or stab injury was found in the observation group. The stone removal rates of observation group and control group were 95.45% (21 / 22) and 91.33% (21 / 23) respectively. Conclusion Point-touch probe lithotripsy has the advantages of high speed, high efficiency, good safety and good clinical application value.
