EpCAM在前列腺癌及前列腺上皮内瘤中的表达及其意义

【摘要】:目的:探讨在前列腺癌(PCa)及前列腺上皮内瘤(PIN)中上皮特异性粘附分子(EpCAM)的表达及其临床意义;探讨高NLR值是否与PCa的恶性度及骨转移相关。 方法:采用免疫组织化学染色法观察EpCAM在63例PCa、46例PIN及58例癌旁良性组织的上皮细胞及间质细胞中的表达情况;采集110例PCa患者术前外周静脉血,计算NLR值,分析术前NLR值与患者民族、年龄、血小板计数、PCa病理类型、骨转移、血清PSA值之间的关系。 结果:(1) EpCAM在PCa和癌旁PIN的上皮细胞中阳性表达率高于癌旁良性腺体(P0.01)。(2) EpCAM在PCa的间质细胞中阳性表达率高于在癌旁PIN(P0.01)。(3) EpCAM在PCa高中分化组和低分化组间的表达差异无统计学意义(P0.05)。(4) EpCAM在有骨转移组和无骨转移组的间质细胞表达差异具有统计学意义(P0.05)。(5)血清PSA值在高NLR组与低NLR组之间的差异具有统计学意义(P0.05)。 结论:EpCAM在PCa的间质细胞中的表达与浸润及骨转移有关,有望成为PCa骨转移的预测指标,进一步研究EpCAM在EMT过程中的作用,为PCa的靶向基因治疗提供理论依据。
[Abstract]:Objective: to investigate the expression and clinical significance of epithelial-specific adhesion molecule (EpCAM) in prostate cancer (PCa) and prostatic intraepithelial tumor (PIN), and to explore whether high NLR value is related to the malignancy of PCa and bone metastasis. Methods: immunohistochemical staining was used to observe the expression of EpCAM in epithelial cells and interstitial cells of 63 cases of PCa,46 and 58 cases of benign tissues adjacent to carcinoma. The peripheral venous blood was collected from 110 patients with PCa before operation and the NLR value was calculated. The relationship between preoperative NLR value and patient's nationality, age, platelet count, PCa pathological type, bone metastasis and serum PSA value was analyzed. Results: (1) the positive expression rate of EpCAM in the epithelial cells of PCa and adjacent PIN was higher than that in the stromal cells of PCa (P0.01). (2) EpCAM was higher than that in adjacent PIN (P0.01). (3). There was no significant difference in the expression of EpCAM between PCa high differentiation group and low differentiation group (P0.05). (4) the expression of EpCAM in interstitial cells of bone metastasis group and non-bone metastasis group had statistical significance (P0.05). (). 5) the difference of serum PSA between high NLR group and low NLR group was statistically significant (P0.05). Conclusion: the expression of EpCAM in the interstitial cells of PCa is related to invasion and bone metastasis, which is expected to be a predictor of PCa bone metastasis. Further study on the role of EpCAM in the process of EMT provides a theoretical basis for PCa targeted gene therapy.
