终末期肾脏病患者血清ucMGP及PIVKA-Ⅱ与桡动脉钙化的相关性研究

【摘要】:目的观察终末期肾脏病患者血清非活性基质Gla蛋白(uncarboxylated matrixGla protein,ucMGP)水平及桡动脉钙化情况,并探讨二者之间的关系;通过凝血酶原前体蛋白(protein induced by vitamine K absence or antagonist-Ⅱ,PIVKA-Ⅱ)的检测评估终末期肾脏病患者体内维生素K状态;初步探讨维生素K缺乏状态对ucMGP及桡动脉钙化的影响。方法选取2012年6月至2013年5月安徽医科大学附属省立医院肾脏内科终末期肾脏病住院患者45例,行前臂桡动脉-头静脉端端吻合术时取桡动脉标本,桡动脉标本均行钙盐染色,,判断有无钙化,并计算钙化积分。另设对照组30例,来自我院健康体检中心。所有参加者均抽取空腹血标本检查:采用血细胞分析仪测定血红蛋白(hemoglobin,Hb)水平,采用全自动生化分析仪测定血钙(calcium,Ca)、血磷(phosphorus,P)、白蛋白(albumin,ALB),应用电化学发光法测定血清全段甲状旁腺素(intact parathyroid hormone,iPTH)、C反应蛋白(Creactive protein,CRP)。采用酶联免疫吸附技术(enzyme linked immunosorbentassay,ELISA)测定血清ucMGP及PIVKA-Ⅱ水平。 结果临床血液指标检测病例组血Hb、Ca、ALB低于对照组(P<0.01),P、iPTH、CRP高于对照组(P<0.01)。病例组血清ucMGP为(198.96±93.96)nmol/L,对照组为(468.17±191.90)nmol/L,病例组低于对照组(P<0.01)。病例组血清PIVKA-Ⅱ为(4.79±4.05)μg/L,对照组为(1.92±1.59)μg/L,病例组高于对照组(P<0.01)。病例组45例桡动脉标本中,钙化均发生在血管中膜。无钙化的28例(62.2%),轻度钙化的12例(26.7%),重度钙化的5例(11.1%)。桡动脉无钙化组、轻度钙化组及重度钙化组3组间比较血ucMGP、血P水平差异有统计学意义(P<0.01)。LSD法组间比较提示重度钙化组血清PIVKA-Ⅱ与轻度钙化组PIVKA-Ⅱ差异有统计学意义(P<0.05)。Spearman相关分析提示病例组桡动脉钙化程度与ucMGP呈负相关(r=-0.64,P<0.05),与血清P、iPTH、CRP呈正相关。Pearson相关分析提示ucMGP与血清P呈负相关(rs=-0.31,P<0.05),血清ucMGP与PIVKA-Ⅱ无明显相关性。 结论终末期肾脏病患者血清ucMGP低于正常人群,且与桡动脉中膜钙化相关。终末期肾脏病患者血清PIVKA-Ⅱ高于正常人群,该人群存在维生素K缺乏。终末期肾脏病患者血清ucMGP水平与PIVKA-Ⅱ水平无明显相关性。
