改良拖入式输尿管肠管吻合在恶性膀胱肿瘤患者原位回肠膀胱术中的应用

【摘要】：目的观察改良拖入式输尿管肠管吻合在恶性膀胱肿瘤患者原位回肠膀胱术中的应用效果。方法收集恶性膀胱肿瘤患者79例,均行膀胱根治性切除术+原位回肠膀胱术;其中37例(A组)采用肠管外黏膜下隧道式输尿管肠管吻合方法进行原位回肠膀胱术,42例(B组)采用改良拖入式输尿管肠管吻合方法进行原位回肠膀胱术。结果两组均手术顺利,A组吻合输尿管与肠管所需时间为(27.6±3.4)min,B组为(5.2±0.5)min;两组相比较,B组吻合输尿管与肠管所需时间短于A组(P㩳0.05)。A组术后并发吻合口瘘2例、吻合口狭窄3例、尿液反流2例、尿路感染2例,并发症发生率为24.32%;B组术后并发吻合口瘘1例、尿路感染2例,并发症发生率为7.14%。两组并发症发生率比较,P㩳0.05。结论在恶性膀胱肿瘤患者原位回肠膀胱术中,改良拖入式输尿管肠管吻合方法能明显缩短手术时间,降低术后泌尿系统并发症的发生。
