非肌层浸润性膀胱癌术后复发的相关因素分析

【摘要】：目的：探讨影响非肌层浸润性膀胱癌患者行经尿道膀胱肿瘤电切术后复发的相关因素。 方法：回顾性分析2009年4月至2013年10月在福建医科大学附属第一医院泌尿外科手术治疗的159例非肌层浸润性膀胱癌患者的临床和病理资料，选择9项可能影响术后复发的指标，用SPSS19.0软件进行单因素和多因素分析，，筛选对非肌层浸润性膀胱术后复发有影响的临床病理因素。 结果：到随访期止，159例患者中29例术后复发，术后总复发率为18.24%，平均无复发生存时间为21.25个月。单因素分析表明影响非肌层浸润性膀胱术后复发的因素为肿瘤大小、组织学分级、既往病史、肿瘤分期、肿瘤数目（P＜0.05），多因素COX分析提示肿瘤分期（HR=2.741）、肿瘤数目(HR=2.293)是影响术后复发的独立因素。 结论：肿瘤大小、组织学分级、既往病史、肿瘤分期、肿瘤数目是非肌层浸润性膀胱癌术后复发的相关因素。其中肿瘤分期、肿瘤数目是复发的独立影响因素。
[Abstract]:Objective: to investigate the related factors of recurrence after transurethral resection of bladder tumor in patients with non-myenteric invasive bladder cancer. Methods: the clinical and pathological data of 159 patients with non-myenteric invasive bladder cancer treated by urology in the first affiliated Hospital of Fujian Medical University from April 2009 to October 2013 were analyzed retrospectively. nine indexes that might affect postoperative recurrence were selected. Univariate and multivariate analysis was carried out by SPSS19.0 software to screen the clinicopathological factors affecting the postoperative recurrence of non-myenteric invasive bladder. Results: at the end of the follow-up period, 29 of 159 patients had recurrence after operation, the total recurrence rate was 18.24%, and the average survival time without recurrence was 21.25 months. Univariate analysis showed that the factors affecting the recurrence of non-myenteric invasive bladder were tumor size, histologic grade, previous history, tumor stage and tumor number (P < 0.05). Multivariate COX analysis showed tumor staging (HR=2.741). Tumor number (HR=2.293) is an independent factor affecting postoperative recurrence. Conclusion: tumor size, histologic grade, previous history, tumor stage and tumor number are related factors for postoperative recurrence of non-myenteric invasive bladder cancer. Among them, tumor stage and tumor number are independent influencing factors of recurrence.
