无糖尿病视网膜病变糖尿病患者黄斑区视网膜厚度与病程及血糖水平的相关性研究

【摘要】:目的分析正常人群与糖尿病患者未发生视网膜病变时黄斑区视网膜厚度差异,以及糖尿病患者黄斑区视网膜厚度与糖尿病病程以及血糖水平的关系。方法选取2016年4月至11月于我院内四科治疗的糖尿病无视网膜病变患者(22例)为糖尿病无视网膜病变组,同期另选22名正常人为正常组。光学相干断层扫描测定所有患者的黄斑中心凹视网膜厚度(central subfield retinal thickness,CSRT)、黄斑部体积(cube volume,CV)及平均黄斑区厚度(cube average thickness,CAT),分析糖尿病患者与正常人群CSRT、CV、CAT的差别。将糖尿病无视网膜病变患者按照糖尿病病程(分为A、B、C 3组)及血糖状态(分为D、E 2组)进行分组,比较组间CSRT、CV、CAT差异。结果糖尿病无视网膜病变组患者随着糖尿病病程的增加CSRT、CV及CAT逐渐增加,差异均有统计学意义(均为P0.05)。糖尿病无视网膜病变组患者随着糖尿病患者血糖的上升CSRT、CV及CAT逐渐增加,差异均有统计学意义(均为P0.05)。CSRT、CV及CAT与糖尿病病程及血糖均呈正相关(均为P0.05)。结论糖尿病患者相比正常人群CSRT、CV及CAT明显增厚,且CSRT、CV及CAT随糖尿病病程及血糖的增加而逐渐升高,为糖尿病早期视网膜病变的诊断提供可靠的参考。
[Abstract]:Objective to analyze the difference of retinal thickness in macular area between normal subjects and diabetic patients without retinopathy. Methods from April 2016 to November, 22 cases of diabetic non-retinopathy treated in our hospital were selected as diabetic non-retinopathy group. In the same period, 22 normal subjects were selected as normal subjects. Central subfield retinal thicknessnessct, cube volume and mean macular thickness of macular area were measured by optical coherence tomography (Oct) in all patients. The mean macular thickness and mean macular thickness were analyzed between diabetic and positive diabetic patients. According to the course of diabetes (group A BX C 3) and blood glucose status (group D 2), the patients with diabetic retinopathy were divided into two groups. Results the CV and CAT of diabetic patients without retinopathy increased with the increase of the course of diabetes. The difference was statistically significant (P 0.05). CSRT CV and CAT increased with the increase of blood glucose in diabetic patients without retinopathy. The difference was statistically significant (P0.05N. CSRTCV and CAT were positively correlated with the course of diabetes and blood glucose (P0.05). Conclusion compared with the normal population, the CV and CAT of diabetic patients were significantly thicker, and the CV and CAT increased gradually with the increase of the course of diabetes and the increase of blood glucose. To provide a reliable reference for the diagnosis of early diabetic retinopathy.
