青少年双生子全血脂肪酸组成与空腹血脂水平关系的横断面研究

【摘要】:青少年时期的血浆甘油三酯水平升高、血脂代谢异常被广泛认为是成年后的肥胖、代谢综合症和心血管疾病的早期症候。研究表明空腹血脂水平与膳食脂肪的摄入密切相关。目前,针对中国青少年儿童膳食脂肪摄入情况与空腹血脂水平的研究相对较少,而且由于研究对象的遗传基础存在差异以及其受后天环境的影响不同,以往相关的研究结果存在不一致。本研究纳入了浙江嘉兴地区的311对7-15周岁的健康青少年双生子,测定研究对象的全血脂肪酸组成,将其作为膳食脂肪摄入的生物标记物,探讨了研究中国青少年膳食脂肪摄入情况与空腹血脂水平的关系。同时利用双生子研究的特点,即双生子分为遗传基础完全相同的单卵双生子和在遗传上平均有50%相同的异卵双生子,成长在同一个家庭的一对双生子,其受环境因素的影响也相同,我们进一步探讨了青少年儿童全血脂肪酸组成与空腹血脂水平的关系是否受共同的遗传和环境因素的影响。结果显示,在排除了年龄、腰围升高比、卵型、体育锻炼情况、性成熟度和每日总摄入能量等因素后,青少年双生子全血多不饱和脂肪酸含量,尤其是二十二碳六烯酸(DHA)的含量与空腹血浆甘油三酯浓度呈显著负相关。针对单卵双生子的对内关联性(within-pair association)分析显示上述关系依然成立,这表明青少年双生子全血DHA含量与空腹甘油三酯浓度的关系不受共有的遗传和环境因素的影响。在本研究中,未发现青少年全血脂肪酸组成与总胆固醇、高密度脂蛋白胆固醇以及低密度脂蛋白胆固醇之间存在显著相关或关联。结果同时也提示,增加膳食中多不饱和脂肪酸,特别是DHA的摄入,可能是防止青少年高甘油三酯血症的一个保护因素。
[Abstract]:Elevated plasma triglyceride levels and dyslipidemia in adolescence are widely thought to be obesity in adulthood. Metabolic syndrome and early signs of cardiovascular disease. Studies have shown that fasting blood lipid levels are closely related to dietary fat intake. Because of the difference of genetic basis and the influence of acquired environment, the results of previous studies are inconsistent. This study included 311 twins of healthy adolescents aged 7-15 in Jiaxing, Zhejiang Province. The fatty acid composition of whole blood was measured and used as a biomarker of dietary fat intake. The relationship between dietary fat intake and fasting blood lipid level in Chinese adolescents was studied. At the same time, the characteristics of Gemini study were used to study the relationship between dietary fat intake and fasting blood lipid level. That is, the twins are divided into monozygote twins with identical genetic basis and heterozygote twins with an average of 50% identical in heredity, and a pair of twins growing up in the same family are equally affected by environmental factors. We further explored whether the relationship between fatty acid composition in whole blood and fasting blood lipid level in adolescents and children was affected by common genetic and environmental factors. After sex maturity and total daily energy intake, the whole blood polyunsaturated fatty acids content in twins of adolescents, In particular, the content of 22 carbohexaenoic acid was negatively correlated with fasting plasma triglyceride concentration. The results indicated that the relationship between serum DHA content and fasting triglyceride concentration was not affected by common genetic and environmental factors. In this study, no fatty acid composition and total cholesterol were found in the whole blood of adolescents. There was a significant correlation or association between high density lipoprotein cholesterol and low density lipoprotein cholesterol. The results also suggested an increase in the intake of polyunsaturated fatty acids, especially DHA, in the diet. It may be a protective factor against hypertriglyceridemia in adolescents.
