宁夏城市地区回汉族中老年人群睾酮、雌二醇水平与血尿酸代谢相关性研究

【摘要】:目的1.了解宁夏城市地区回、汉族中老年人高尿酸血症患病率情况并对比;2.分析中老年人群雌二醇、睾酮与血尿酸水平的相关性;3.分析影响中老年人群血尿酸水平的相关因素。方法采用整群抽样的流行病学调查方法,调查2011年8月-2012年12月银川市、吴忠市五个不同社区人群1295人(回汉比例为1:1.91),对调查人群进行问卷调查、体格检查,并检测尿酸(UA)、睾酮(T)、雌二醇(E2)、空腹胰岛素(INS)、性激素结合球蛋白(SHBG)、总胆固醇(TC)、甘油三酯(TG)、空腹血糖(FBG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)等指标,采用SPSS17.0软件进行数据分析。结果1.男性睾酮65岁以后随年龄有降低趋势(F=7.146,P=0.000),女性雌二醇水平55岁以后呈降低趋势(F=5.315,P=0.001)。2.回族高尿酸血症患病率为7.87%;汉族高尿酸血症患病率为12.47%,汉族中老年人高尿酸血症患病率高于回族(2x=6.385,P=0.011)。3.男性高尿酸血症患病率17.82%,女性高尿酸血症患病率4.39%,男性患病率高于女性(2x=59.839,P=0.000)。各年龄组男性均高于女性(P0.05)。4.回、汉族人群高血压组的高尿酸血症患病率均高于正常血压组(P0.05)。5.在线性回归分析中,血尿酸随睾酮(B=0.037,P=0.000)、空腹血糖(B=2.475,P=0.011)、甘油三酯(B=10.066,P=0.000)、收缩压(B=0.379,P=0.001)的升高而升高,随HDL-C(B=-8.706,P=0.000)的升高而降低。结论1.高尿酸血症患病率在回汉民族间、男女性别间存在差异。2.高血压、空腹血糖升高、血脂代谢紊乱与血尿酸水平相关。3.高密度脂蛋白与血尿酸水平呈负相关。4.机体睾酮水平与血尿酸呈正相关。该研究提示:高尿酸血症的患病率与性别、血压和甘油三酯水平有关,预防高尿酸血症应从控制血压、降低甘油三酯水平着手。
[Abstract]:Objective 1. To understand the prevalence of hyperuricemia in middle and middle aged people in urban areas of Ningxia and to compare the prevalence of hyperuricemia. The correlation between testosterone and serum uric acid levels. 3. Analysis of the related factors affecting the serum uric acid level in the middle-aged and elderly population. Methods the epidemiological survey method of cluster sampling was used to investigate Yinchuan City from August 2011 to December 2012. In Wuzhong City, 1295 people from five different communities (the ratio of returning to Han is 1: 1.91) were investigated by questionnaire and physical examination. The indexes of UAA, testosterone, estradiol, estradiol, fasting insulin, sex hormone binding globulin, total cholesterol, triglyceride, fasting blood glucose, high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C) were detected. SPSS17.0 software was used to analyze the data. 1. Male testosterone decreased with age after 65 years of age, and female estradiol level decreased after 55 years. The prevalence of hyperuricemia was 7.87% in Hui nationality and 7.87% in Han nationality. The prevalence rate of hyperuricemia was 12.47. The prevalence rate of hyperuricemia in the Han nationality was higher than that in the Hui nationality. The prevalence rate of hyperuricemia was 17.82% in males and 4.39 in females. The prevalence rate of hyperuricemia in males was higher than that in females (2x59.839 P0.000). The prevalence of hyperuricemia in males was higher than that in females. The prevalence rate of hyperuricemia in the hypertension group of Han nationality was higher than that in the normal blood pressure group (P 0.05). In linear regression analysis, the serum uric acid was increased with the increase of testosterone (0.037), fasting blood glucose (B) 2.475p (0.011), triglyceride (10.066P0.000), systolic blood pressure (B0.379p 0.001). Conclusion 1. The prevalence rate of hyperuricemia is different between male and female in the Hui Han nationality. 2. Hypertension, fasting blood glucose increase, the incidence of hyperuricemia is decreased with the increase of HDL-Con (8. 706) (P < 0. 000). 1. The prevalence of hyperuricemia is different between male and female. High density lipoprotein (HDL) has a negative correlation with serum uric acid level. 4. There is a positive correlation between serum testosterone level and serum uric acid level. This study suggests: prevalence and sex of hyperuricemia. Blood pressure is related to triglyceride level. To prevent hyperuricemia, we should control blood pressure and reduce triglyceride level.
