系统性红斑狼疮患者血液系统受累特点及临床意义

【摘要】:目的:分析系统性红斑狼疮(systemic lupus erythematosus,SLE)患者血液系统受累的特点,并探讨其临床意义。方法:按照国家风湿病数据中心要求收集并分析334例SLE患者的临床资料,将患者分为有血液系统受累组和无血液系统受累组,分析血液系统受累与SLE临床指标、实验室指标、疾病活动度等相关性。数据采用SPSS20.0进行统计分析。结果:(1)334例SLE患者纳入研究范围,其中女性患者311例,男性患者23例。SLE患者中,213例(63.8%)患者合并血液系统受累,其中贫血占49.1%,白细胞减少占32.0%,血小板减少占18.6%;单系血细胞减少占35.9%,两系及两系以上受损占27.8%。(2)与无血液系统受累组比较,血液系统受累组皮疹、黏膜溃疡及浆膜炎的比例明显增多,差异具有显著性(P0.05)。(3)SLE患者中,血液系统受累组红细胞沉降率升高、补体C3降低、抗双链DNA抗体阳性、抗心磷脂抗体阳性率明显高于无血液系统受累组,差异具有统计学意义(P0.05)。(4)血液系统受累组疾病轻、中和重度活动例数与无血液系统受累组比较明显增多,差异均有统计学意义(P0.05)。结论:血液系统是SLE患者最常受累的系统之一,以贫血最为常见;合并血液系统受累SLE患者免疫学指标明显异常,病情活动更明显。
[Abstract]:Objective: to analyze the characteristics of blood system involvement in patients with systemic lupus erythematosus (lupus) and to explore its clinical significance. Methods: according to the requirements of National Rheumatological data Center, the clinical data of 334 patients with SLE were collected and analyzed. The patients were divided into two groups: the group with blood system involvement and the group without blood system involvement. The clinical indexes and laboratory indexes of blood system involvement and SLE were analyzed. Data were analyzed by SPSS20.0. Results 1334 patients with SLE were included in the study, including 311 female patients and 213 cases of male patients (63.8%). Anemia accounted for 49.1%, leukopenia accounted for 32.0%, thrombocytopenia accounted for 18.66.Monocytocytopenia accounted for 35.9%, and damage of two or more lines accounted for 27.80.2.Compared with the group without involvement of the blood system, the hematological system involved group had rash. The percentage of mucosal ulcers and serositis increased significantly. The erythrocyte sedimentation rate increased, complement C3 decreased and anti-double-stranded DNA antibody was positive in the blood system involvement group. The positive rate of anticardiolipin antibody was significantly higher than that of the group without involvement of blood system, the difference was statistically significant (P 0.05. 05. 4) the number of moderate and severe activity in the group of blood system involvement was significantly higher than that in the group without involvement of blood system. Conclusion: the blood system is one of the most frequently involved systems in patients with SLE, anemia is the most common, and the immunological indexes in patients with SLE complicated with hematological system involvement are obviously abnormal, and the disease activity is more obvious.
