益肾补骨汤联合椎体成形术治疗老年骨质疏松性胸腰椎压缩骨折的临床疗效及对患者生存质量的影响

【摘要】：目的探讨益肾补骨汤联合椎体成形术治疗老年骨质疏松性胸腰椎压缩骨折的临床效果及对生存质量的影响。方法选择该院接受治疗的100例老年骨质疏松性胸腰椎压缩骨折患者,采取计算机单盲分组法随机分为两组,对照组50例采取椎体成形术治疗,观察组50例采取椎体成形术联合益肾补骨汤治疗,比较两组治疗效果、疼痛评分、Oswestry功能障碍指数(ODI)、伤椎前后缘高度、后凸角度、骨密度、生存质量评分、再次骨折发生率。结果观察组治疗优良率高于对照组(P0.05)。手术后3个月,两组疼痛评分、ODI明显降低,伤椎前后缘高度、骨密度明显增高,后凸角度明显减小,生存质量评分明显增高,且观察组均明显优于对照组(均P0.05)。随访1年发现,观察组的再次骨折发生率明显低于对照组(P0.05)。结论椎体成形术联合益肾补骨汤治疗老年骨质疏松性胸腰椎压缩骨折具有显著的临床效果,可有效缓解骨折疼痛,恢复伤椎高度和椎体后凸角度,改善胸腰椎活动功能,还能有效避免再次发生骨折,进而提高患者的生存质量。
[Abstract]:Objective to investigate the clinical effect of Yishen Bugu decoction combined with vertebroplasty in the treatment of osteoporotic thoracolumbar compression fracture and its effect on quality of life.Methods 100 elderly patients with osteoporotic thoracolumbar compression fractures were randomly divided into two groups, 50 patients in the control group were treated with vertebroplasty.In the observation group, 50 cases were treated with vertebroplasty combined with Yishen Bugu decoction. The therapeutic effects were compared between the two groups. Pain score and Oswestry dysfunction index (Oswestry dysfunction index), height of anterior and posterior edge of injured vertebrae, kyphoid angle, bone mineral density, quality of life score and the incidence of refracture were compared between the two groups.Results the excellent and good rate of treatment in the observation group was higher than that in the control group (P 0.05).At 3 months after operation, the pain score and ODI of the two groups were significantly decreased, the height of the anterior and posterior edge of the injured vertebrae, bone mineral density, kyphosis angle and quality of life were significantly increased, and the observation group was significantly better than the control group (all P 0.05).It was found that the incidence of refracture in the observation group was significantly lower than that in the control group for one year.Conclusion Vertebroplasty combined with Yishen Bu decoction in the treatment of senile osteoporotic thoracolumbar vertebral compression fracture has significant clinical effect, which can effectively relieve fracture pain, recover the height of injured vertebrae and kyphosis angle, and improve the function of thoracolumbar vertebrae.Can also effectively avoid the recurrence of fracture, and then improve the quality of life of patients.
