北京地区55岁以上男性2型糖尿病患者肌少症与骨密度的相关性研究

【摘要】:目的通过检测北京地区55岁以上男性糖尿病肌少症患者与糖尿病肌量正常患者的四肢骨骼肌质量(appendicular skeletal muscle mass,ASM)、骨密度(bone mineral density,BMD)及相关因素,调查中老年男性2型糖尿病(Type 2 diabetes mellitus,T2DM)患者肌少症(Sarcopenia)的患病情况,探讨2型糖尿病患者骨骼肌肉质量减少与骨密度的相关性,促进健康生活习惯的养成,实现对中老年T2DM患者肌少症及骨质疏松症的二级预防和早期干预方法选取首都医科大学宣武医院内分泌科2016年5月至12月,119名55-87岁男性T2DM住院患者,记录受试者一般情况,用双光能X线吸收仪测量腰椎、左侧股骨颈、右侧股骨颈BMD,ASM,四肢脂肪含量(appendicular fat mass,AFM)等,计算出四肢骨骼肌质量指数(appendicular skeletal musclemass index,ASMI),通过测量握力方法评价肌力,采用6米步行测试评估受试者的躯体功能,采用1998年Baumgartner诊断标准定义肌少症。采用线性相关分析方法统计ASM、AFM、ASMI、肌力等与BMD的相关性,采用Logistic回归分析统计肌少症与骨质疏松症的相关性。结果1、北京地区55岁以上男性T2DM患者肌少症检出率为28.57%,低骨骼肌含量(ASMI7.26kg/m2)的55岁以上男性T2DM患者BMD明显低于非低骨骼肌含量(ASMI≥7.26kg/m2)者。2、相关分析显示ASMI与腰椎BMD及左、右股骨颈BMD均成正相关,T2DM低骨骼肌含量受试者患骨质疏松症的风险明显增加,是T2DM非低骨骼肌含量受试者的3.067倍(OR=3.067,95%CI:1.023~9.190,P=0.045)。结论北京地区55岁以上男性T2DM患者的骨骼肌质量减少与骨质疏松症密切相关。中老年男性T2DM患者应及时进行肌少症及骨质疏松症的早期筛查以实现早期预防和及时干预。
[Abstract]:Objective to investigate the Beijing area men over the age of 55 diabetic patients with normal muscle in patients with diabetes and less muscle mass limbs skeletal muscle mass (appendicular skeletal muscle mass, ASM), bone mineral density (bone mineral density, BMD) and related factors among elderly men with type 2 diabetes mellitus (Type 2 diabetes mellitus, T2DM) in patients with muscle less disease (Sarcopenia) prevalence of skeletal muscle in patients with type 2 diabetes to reduce the quality of correlation with bone density, promote healthy lifestyle, to achieve two levels of prevention and early intervention methods less disease and osteoporosis in elderly patients with T2DM muscle from Capital Medical University Xuanwu Hospital Department of endocrinology from May 2016 to December, 119 A 55-87 year old male patients with T2DM were recorded in general, measured by dual energy X-ray absorption of lumbar spine, left femoral neck, right femoral neck bone BMD, ASM, limb fat containing The amount of (appendicular fat mass, AFM), calculate the limb skeletal muscle mass index (appendicular skeletal musclemass index, ASMI), by measuring the grip strength evaluation method of strength, the 6 meter walk test subjects were evaluated by the physical function, the 1998 Baumgartner diagnostic standard definition of sarcopenia. Using correlation analysis method of linear statistics ASM AFM, ASMI, and BMD, correlation of muscle strength, using Logistic correlation analysis statistical sarcopenia and osteoporosis. The regression results in 1, the Beijing area over 55 years old male T2DM patients with sarcopenia detection rate was 28.57%, the low content of skeletal muscle (ASMI7.26kg/m2) of the 55 men over the age of T2DM patients was significantly lower than that of non BMD low content of skeletal muscle (ASMI = 7.26kg/m2).2, correlation analysis showed that ASMI and BMD of the lumbar spine and left and right femoral neck BMD were positively related to T2DM risk, low content of skeletal muscle in subjects suffering from osteoporosis increased significantly, T2DM is 3.067 times of non low skeletal muscle content of subjects (OR=3.067,95%CI:1.023 ~ 9.190, P=0.045). 55 years old male T2DM patients in Beijing conclusion skeletal muscle mass reduction is closely related with osteoporosis. T2DM in elderly male patients should be early screening of sarcopenia and osteoporosis in order to achieve early the prevention and timely intervention.
