狼疮肾炎活动与IL-17水平相关性研究

【摘要】：目的:检测狼疮肾炎(lupus nephritis,LN)患者血清和肾组织的白介素17(Interleukin 17,IL-17)表达水平,结合临床相关指标,分析IL-17水平与疾病活动度、肾脏病理损害程度等的相关性,初步探讨IL-17在LN发病与发展中的作用。方法:采用免疫组化法检测54例LN患者肾组织IL-17表达水平,酶联免疫吸附测定法(enzyme-linked immunosorbent assay,ELISA)检测26例LN患者和30例年龄、性别均匹配的健康对照者血清IL-17水平,并对结果进行统计学分析。结果:1.Ⅳ型LN患者肾组织的IL-17表达水平明显高于Ⅲ型、Ⅴ型、Ⅲ+Ⅴ型、Ⅳ+Ⅴ型,差异有统计学意义。2.LN患者肾组织IL-17表达水平与肾脏病理急性指数(r=0.547,P0.001)、尿蛋白(r=0.446,P=0.001)、24h尿蛋白定量(r=0.399,P=0.003)呈正相关,与肾脏慢性指数无相关性。3.LN患者血清IL-17水平高于健康对照组,差异有统计学意义。4.根据疾病活动指数将LN组分为LN活动组(11例)、LN非活动组(15例),LN活动组血清IL-17水平明显高于LN非活动组和健康对照组,LN非活动组IL-17水平高于健康对照组,差异均有统计学意义。5.LN患者血清IL-17水平与疾病活动指数呈正相关(r=0.640,P0.001),与补体C3水平呈负相关(r=-0.410,P=0.0026),与补体C4无相关性。结论:IL-17在LN的发病和发展中发挥一定作用。
