百草枯中毒肺损伤CT表现及其病理基础的实验研究

【摘要】:目的:研究百草枯(paraquat,PQ)中毒所致肺损伤的胸部CT表现,提高对百草枯中毒的认识。方法:回顾性对比与分析122例百草枯中毒患者的临床与胸部CT表现,按中毒剂量[1]将患者分为少量组(10 ml)46例、中量组(11~50 ml)56例、大量组(50 ml)20例,按病程[1]将患者分为早期(1~7 d)、中期(8~14 d)、晚期(14 d以后)三组,通过对比不同组的CT表现同剂量与时间的关系、分析病变分布范围,总结百草枯中毒的影像学特点。结果:随着时间进展[1],百草枯中毒患者胸部CT具有特征性,早期表现为肺纹理增多、磨玻璃征,然后逐渐出现渗出实变,最后肺纤维化[1],并且与中毒剂量相关(P0.05);肺内渗出与实变病灶主要分布于双肺野外带,为临床诊疗提供依据。结论:百草枯中毒肺损伤的胸部CT所见病变转变过程有一定特征性[1],其损伤程度与剂量、时间相关,对临床诊疗具有十分重要的意义[1]。目的:建立急性百草枯中毒肺损伤大鼠模型,为进一步探讨百草枯的中毒机制和治疗效果奠定基础。方法:SD大鼠共60只,雌雄各半,随机分为3个实验组和1个对照组,每组15只,分别给大鼠一次性腹腔注射不同剂量的百草枯溶液,对照组一次性腹腔注射等体积的无菌等渗盐水,每天观察大鼠中毒症状及CT表现,并于1、3、7、14、21 d分别处死3只实验组及对照组大鼠,取大鼠右下叶肺组织做病理切片染色观察。结果:一次性腹腔注射不同剂量(40 mg/kg、60 mg/kg、80 mg/kg)的百草枯溶液后,大鼠在不同时间段出现了不同程度的中毒症状,光镜下可观察到大鼠百草枯中毒后肺损伤的病理演变过程。结论:百草枯对大鼠具有强毒性,一次性腹腔注射百草枯溶液,可成功建立肺损伤模型。
[Abstract]:Objective: to study the chest CT findings of lung injury caused by paraquat PQQ poisoning and to improve the understanding of paraquat poisoning. Methods: the clinical and thoracic CT findings of 122 paraquat poisoning patients were retrospectively compared and analyzed. According to the dose of poisoning [1], the patients were divided into a small group of 10 ml)46, a moderate dose group of 1150 cases and a large number of patients with 50 ml)20. According to the course of disease [1], the patients were divided into three groups: early stage (1 day), middle stage (814 d), late stage (14 d). By comparing CT findings of different groups with the same dose and time, the range of lesion distribution was analyzed and the imaging features of paraquat poisoning were summarized. Results: with the development of time, the chest CT of paraquat poisoning patients was characterized by the increase of lung texture, grinding of glass sign, and then the appearance of exudation and consolidation. Finally, pulmonary fibrosis was found [1], which was related to the dose of poisoning (P0.05). The focus of exudation and consolidation in the lung was mainly distributed in the field zone of both lungs, which provided the basis for clinical diagnosis and treatment. Conclusion: the change process of chest CT findings of paraquat poisoning lung injury is characteristic [1]. The degree of injury is correlated with dose and time, which is of great significance for clinical diagnosis and treatment. Objective: to establish a rat model of acute paraquat poisoning and to establish a foundation for further study on the mechanism and therapeutic effect of paraquat poisoning. Methods Sixty Sprague-Dawley rats, half male and half male, were randomly divided into 3 experimental groups and 1 control group, 15 rats in each group were injected with paraquat solution in a single dose. The rats in the control group were injected intraperitoneally with iso-osmotic saline of the same volume once a day. The toxic symptoms and CT findings of the rats were observed every day. Three rats in the experimental group and the control group were killed for 21 days. The lung tissue of the right lower lobe of the rats was stained with pathological sections. Results: after intraperitoneal injection of different doses of 40 mg / kg ~ 60 mg / kg ~ (80 mg 路kg ~ (-1) paraquat solution, the rats showed different degrees of poisoning symptoms in different time periods. The pathological process of lung injury after paraquat poisoning in rats was observed under light microscope. Conclusion: paraquat has strong toxicity to rats. Once intraperitoneal injection of paraquat solution can successfully establish lung injury model.
