合并2型糖尿病的结肠癌的临床特征分析及EMT相关蛋白E-cadherin和MMP9表达的研究

【摘要】:目的:研究合并T2DM的结肠癌患者的年龄、性别、高血压情况、TNM分期、肿瘤浸润程度、淋巴转移、远处转移与单纯结肠癌患者有无区别,并了解合并T2DM的结肠癌组织中EMT相关蛋白E-cadherin(上皮细胞钙粘蛋白)、MMP9(基质金属蛋白酶9)的表达情况。方法:收集2004年1月-2010年6月在广西医科大学第一附属医院病理科诊断为单纯结肠癌患者48例,经病理科及临床医师共同诊断为合并T2DM的结肠癌患者40例,收集2015年5月-2017年4月在广西医科大学第九附属医院病理科诊断为单纯结肠癌患者27例,经病理科及临床医师共同诊断为合并T2DM的结肠癌患者19例,并收集病人的年龄、性别、高血压情况、TNM分期、肿瘤浸润程度、淋巴转移、远处转移的数据。运用SPSS13.0统计软件进行分析。从广西医科大学第一附属医院收集到的病例中使用Excel随机数列法随机选出单纯结肠癌患者35例,合并T2DM的结肠癌患者35例,另收集同时期病理科诊断为单纯结肠腺瘤患者35例,运用免疫组织化学的方法检测各组标本中的E-cadherin、MMP9的表达情况。结果:1.合并T2DM的结肠癌患者的年龄比单纯结肠癌患者大。2.单纯结肠癌患者与合并T2DM的结肠癌的患者的浸润程度有统计学差异,两者均以T3-T4为主,其中单纯结肠癌患者T3-T4占66.67%,合并T2DM的结肠癌患者T3-T4占83.05%。3.合并T2DM的结肠癌患者比单纯结肠癌患者更易合并高血压(X2=25.382,P=0.000)。4.E-cadherin在单纯结肠腺瘤组、单纯结肠癌组及合并T2DM的结肠癌组的表达中,组间有统计学差异,但对三组进行两两比较均无统计学差异。5.MMP9在单纯结肠腺瘤组、单纯结肠癌组及合并T2DM的结肠癌组的表达中,组间有统计学差异。单纯结肠腺瘤与单纯结肠癌、合并T2DM的结肠癌两两比较均有统计学差异,但单纯结肠癌与合并T2DM的结肠癌之间两两比较无统计学差异。结论:1.合并T2DM的结肠癌的患者的年龄较单纯结肠癌患者高。2.合并T2DM的结肠癌患者侵袭能力较单纯结肠癌患者更强。3.合并T2DM的结肠癌患者与单纯结肠癌患者相比,更易合并高血压。4.MMP9可能是区分单纯结肠腺瘤与单纯结肠癌的指标。5.T2DM影响结肠癌发生发展的过程可能不依赖于E-cadherin、MMP9的作用。
[Abstract]:Objective: to study the age, sex, hypertension stage, tumor invasion, lymphatic metastasis and distant metastasis of colon cancer patients with T2DM. To investigate the expression of E-cadherin (E-cadherin) in human colon carcinoma with T2DM. Methods: from January 2004 to June 2010, 48 patients with simple colon cancer were diagnosed in the Department of Pathology of the first affiliated Hospital of Guangxi Medical University, and 40 patients with T2DM were diagnosed by pathologists and clinicians. From May 2015 to April 2017, 27 patients with simple colon cancer were diagnosed in the Department of Pathology of the Ninth affiliated Hospital of Guangxi Medical University, and 19 patients with T2DM were diagnosed by pathologists and clinicians. The age and sex of the patients were also collected. Data of TNM staging, tumor invasion, lymphatic metastasis and distant metastasis in hypertension. SPSS13.0 statistical software was used to analyze. From the first affiliated Hospital of Guangxi Medical University, 35 patients with simple colon cancer, 35 patients with T2DM and 35 patients with simple adenoma of colon were randomly selected by Excel random sequence method, and 35 patients were diagnosed as simple adenoma of colon by pathology in the same period. The expression of E-cadherinus MMP9 was detected by immunohistochemical method. The result is 1: 1. Colon cancer patients with T2DM are older than those with colon cancer alone. There was a significant difference in the degree of infiltration between the patients with simple colon cancer and those with T2DM. T3-T4 was the main type of infiltration between the two groups. T3-T4 was 66.67 in patients with simple colon cancer, and T3-T4 was 83.05.3. in patients with T2DM, there was a significant difference in the degree of infiltration between patients with simple colon cancer and those with T2DM. Colon cancer patients with T2DM were more likely to be complicated with hypertension than those with simple colon cancer. The expression of E-cadherin in colon adenoma group, colon cancer group and colon cancer group with T2DM was significantly higher than that in simple colon adenoma group, simple colon cancer group and colon cancer group with T2DM. However, there was no significant difference in the expression of MMP9 between the three groups. 5. There was a significant difference in the expression of MMP9 between the three groups in simple colon adenoma group, simple colon cancer group and colon cancer group with T2DM. There was statistical difference between simple colon adenoma and simple colon cancer and colon cancer with T2DM, but there was no statistical difference between simple colon cancer and colon cancer with T2DM. Conclusion 1. Colon cancer patients with T2DM were older than those with colon cancer alone. Colon cancer patients with T2DM were more aggressive. 3. 3 than patients with simple colon cancer. Compared with simple colon cancer patients, patients with T2DM are more likely to be complicated with hypertension. 4. MMP9 may be an index to distinguish simple colon adenoma from simple colon cancer. 5. T2DM may not depend on the role of E-cadherin MMP9 in the development of colon cancer.
