血浆磷脂脂肪酸与人体组成及代谢综合征的关联性研究

【摘要】:研究目的 随着肥胖成为日益严重的健康问题,脂肪与脂肪组织含量的测定及分布成为研究热点。骨骼肌与体内的物质代谢及能量代谢密切相关。代谢综合征是与肥胖相关的一种常见代谢病,代谢综合征高患病率使其成为中国的一个严重公共卫生问题。本研究旨在以杭州社区人群为研究对象,从膳食营养的角度通过测定体内血浆磷脂脂肪酸、脂肪的分布及代谢综合征相关的临床指标,并对脂肪酸与脂肪分布相关指标以及代谢综合征相关临床指标进行分析,以探讨人体脂肪酸的构成与脂肪分布及代谢综合征的关系。 材料与方法 2008-2013年期间在杭州社区招募108名研究对象,用调查问卷收集了他们的社会人口学资料,对他们进行了体格测量(身高、体重和腰围等指标)、磁共振成像(MRI)测量、血液生化指标的测量以及血浆磷脂脂肪酸测量。分别用相关分析、多元线性回归分析和二分类Logistic回归来研究血浆磷脂脂肪酸与人体组成和代谢综合征的关联性。 研究结果 本次研究共有102名研究对象符合要求,研究人群的平均年龄为52.10±±12.55岁,其中男性为51.17±13.75岁,女性为53.23±±10.96岁,两组年龄无统计学差异。男性全身VAT、全身MAT、全身Muscle、腹部VAT、腹部IMAT、腹部Muscle、腰围、收缩压、舒张压、空腹血糖、胆固醇明显较女性高;而全身SAT及腹部SAT在男性人群明显较女性人群低。男性与女性年龄及BMI上无明显差异。在脂肪分布等指标男性与女性有显著性差异。男女性间腰围、收缩压、舒张压、空腹血糖存在显著性差异,甘油三酯、高密度脂蛋白及代谢综合征患病率无显著性差异。男性血浆磷脂脂肪酸C17:0、C20:2相对浓度显著低于女性,C18:3(RLA)、C20:3n3、 C20:4(AA)、C22:4相对浓度显著高于女性,其他种类脂肪酸男性与女性无统计学差异。回归分析结果显示男性人群中,C14:0与全身肌间脂肪呈正相关,C17:0与全身内脏脂肪和全身肌间脂肪呈负相关,C16:0、C18:0、SFA与骨骼肌呈正相关,在女性人群中,脂肪酸与肌间脂肪及骨骼肌无相关性。在脂肪酸与代谢综合征的关系研究中,C12:0、C20:0、C22:6(DHA)与代谢综合征呈负相关,n-6脂肪酸与代谢综合征无显著性相关。 研究结论 1.男性全身VAT、IMAT、Muscle、腹部VAT、IMAT、Muscle较女性高;男性全身SAT及腹部SAT较女性人群低。 2.男性人群中,C14:0含量越高,全身肌间脂肪越多,C17:0含量越高全身内脏脂肪和全身肌间脂肪越少,在女性人群中,脂肪酸与肌间脂肪无相关性; 3.男性人群中,C16:0、C18:0、SFA含量越高,全身骨骼肌含量越多,摄入饱和脂肪酸可以预防肌肉减少症,在女性人群中,脂肪酸与全身骨骼肌无相关性; 4.C12:0、C20:0、C22:6(DHA)含量越高,代谢综合征发病风险越低,n-6脂肪酸与代谢综合征无显著性相关。
[Abstract]:Objective with obesity becoming an increasingly serious health problem, the determination and distribution of fat and adipose tissue has become a hot topic. Skeletal muscle is closely related to substance metabolism and energy metabolism. Metabolic syndrome is a common metabolic disease associated with obesity. The high prevalence of metabolic syndrome makes it a serious public health problem in China. The aim of this study was to determine the distribution of plasma phospholipid fatty acids, fatty acids and related clinical indexes of metabolic syndrome from the point of view of dietary nutrition. The relationship between fatty acid and fatty acid distribution and metabolic syndrome was analyzed in order to explore the relationship between fatty acid composition and fat distribution and metabolic syndrome. Materials and methods 108 subjects were recruited in Hangzhou community from 2008 to 2013. The data of their social demographics were collected by questionnaire, and their physiques were measured (height, height). Weight and waist circumference), magnetic resonance imaging (MRI), blood biochemical parameters and plasma phospholipid fatty acids. The correlation between plasma phospholipid fatty acids and body composition and metabolic syndrome was studied by correlation analysis, multivariate linear regression analysis and binary logistic regression. Results A total of 102 subjects met the requirements. The average age of the study population was 52.10 卤12.55 years old, including 51.17 卤13.75 years for males and 53.23 卤10.96 years for females. There was no significant difference in age between the two groups. Vat, Matt, Muscle. abdominal Vat, abdominal IMATT, abdominal Muscle, waist circumference, systolic blood pressure, diastolic blood pressure, fasting blood glucose and cholesterol were significantly higher in men than in women, while whole body SAT and abdominal SAT were significantly lower in males than in females. There was no significant difference in age and BMI between males and females. There was significant difference in fat distribution between male and female. There were significant differences in waist circumference, systolic blood pressure, diastolic blood pressure and fasting blood glucose between men and women, but there was no significant difference in the prevalence of triglyceride, high density lipoprotein and metabolic syndrome. The relative concentrations of plasma phospholipid fatty acids C17: 0 and C20: 2 in males were significantly lower than those in females, and the relative concentrations of C20: 4 (AA) in C20: 4 were significantly higher than those in females. There was no significant difference in other fatty acids between males and females. Regression analysis showed that there was a positive correlation between C14: 0 and systemic muscle fat in male population. C17: 0 was negatively correlated with visceral fat and intermuscular fat. C16: 0 and C180: 0 SFA were positively correlated with skeletal muscle. There was no correlation between fatty acids and intermuscular fat and skeletal muscle. In the study of the relationship between fatty acids and metabolic syndrome, there was a negative correlation between C12: 0, C20: 0, C22: 6 (DHA) and metabolic syndrome. There was no significant correlation between n-6 fatty acids and metabolic syndrome. Conclusion 1. Male VATT IMATT Muscle. abdominal VAT IMATATM muscle is higher than female, male whole body SAT and abdominal SAT are lower than female. 2. 5%. The higher the content of C14: 0 was in male population, the higher the content of C17: 0 was. The higher the content of C17: 0 was, the lower the content of visceral fat and intermuscular fat was, but there was no correlation between fatty acid and intermuscular fat in female population. The higher the content of SFA in male population is, the higher the content of skeletal muscle is, and the higher the content of skeletal muscle is, the higher the content of DHA (DHA) is, the higher the content of SFA is. The lower the risk of metabolic syndrome, the lower the relationship between n-6 fatty acids and metabolic syndrome.
