2型糖尿病患者晚期糖基化终产物与肾功能的关系

【摘要】:目的:通过分析2型糖尿病(type 2 diabetes mellitus,T2DM)患者晚期糖基化终产物(advanced glycation end products,AGEs)水平与肾功能的关系,为糖尿病肾脏病变(diabetic kidney disease,DKD)的预防及早期诊断提供依据。方法:收集2016.07.01-2017.01.31在山东省立医院内分泌科住院治疗的T2DM患者共532人,根据纳入排除标准,最终共有189例患者进入研究当中,利用糖基化终产物无创检测仪测定皮肤AGEs的水平,并收集血清肌酐、估计肾小球滤过率(estimated glomerular filtration rate,eGFR)、尿白蛋白肌酐比(urinary albumin/creatinine ratio,UACR)及胱抑素C等指标。以皮肤AGEs的四分位数为界点,分别以AGEs水平≤73.3、73.3-81.3、81.3-93.9、≥93.9,将人群分为四组。利用spss17.0软件进行统计学分析,应用单因素方差分析及非参数检验比较组间的差异,并应用多重线性回归分别分析AGEs与肌酐、eGFR、UACR和胱抑素C的相关性。结果:1.本次研究共纳入T2DM患者189例,其中男性101例,女性88例,平均年龄60±11.986岁,糖尿病病程10(11)年,平均皮肤AGEs水平84±14.68。2.四组之间eGFR、胱抑素C差别存在统计学差异,分别为eGFR(107.68 vs.98.61、93.09、89.55,P值分别为0.031、0.000和0.000,均小于0.05),胱抑素C(0.85vs.0.92、1.01、1.13,P值分别为0.007、0.000和0.000,均小于0.05)。四组间log肌酐和logUACR虽然差异没有统计学意义,但随着AGEs升高,两者有逐渐升高的趋势。3.多重线性回归分析显示AGEs水平与eGFR呈负相关关系(beta=-0.252,P=0.001);与胱抑素C呈正相关关系(beta=0.236,P=0.01)。log肌酐与AGEs在单因素线性回归分析时呈正相关关系,而在校正了其他因素之后两者相关性无统计学意义;logUACR在单因素回归分析时与AGEs即无统计学意义,未进一步行多重线性回归分析。结论:AGEs水平与2型糖尿病患者肾功能相关,其水平可以反映肾功能的情况。
[Abstract]:Objective: to analyze the relationship between advanced glycation end product (advanced glycation end products ages and renal function in patients with type 2 diabetes mellitus (type 2 diabetes mellitusus T2DM), so as to provide evidence for the prevention and early diagnosis of (diabetic kidney disease of diabetic nephropathy. Methods: a total of 532 T2DM patients were collected from January 31, 2016.07.01-2017.01.31 in Department of Endocrinology, Shandong Provincial Hospital. According to the exclusion criteria, 189 patients were included in the study. Serum creatinine was collected to estimate glomerular filtration rate (estimated glomerular filtration), urinary albumin creatinine ratio (urinary albumin/creatinine / UACR) and cystatin C.
