超重肥胖患者心脏构型超声数据模型的初步研究

【摘要】:目的:利用数据挖掘技术对超重肥胖患者的临床基本信息及心脏构型常规超声测量指标进行挖掘,揭示出影响因素与心脏构型改变之间的潜在的、有价值的关联信息,分析各指标之间的变化规律,了解心脏构型的动态变化,为临床的辅助诊疗提供依据。方法:选取2015年10月至2016年10月我院住院或体检超重肥胖患者(BMI≥24.0)及正常体重健康人共182例(其中超重患者74例,肥胖患者49例,正常体重患者59例)作为研究对象,排除患有先天性心脏病、心肌病、心脏瓣膜病、严重心律失常等对心脏构型有严重影响的心脏本身疾病的患者,并排除高血压、糖尿病等已被证实对心血管系统有明显影响的慢性病患者。通过HIS及LIS系统获取患者的基本信息(血压、血糖、血脂、肝肾功能、T3、T4、TSH、是否吸烟、是否饮酒、心电图是否异常等基本信息),严格测量患者身高、体重,询问患者的肥胖病程并记录下来。测量心脏构型常规指标(收缩期末的心房、心室内径,心房面积,左室重量,心房、心室容积(辛普森法),左、右室流出道内径,室间隔、左室后壁、右室前壁厚度,主动脉、肺动脉内径,冠状静脉窦内径;舒张期末的心房、心室内径,心房面积,左室重量,心房、心室容积(辛普森法),左、右室流出道内径,室间隔、左室后壁、右室前壁厚度,主动脉、肺动脉内径;吸气末与呼气末的下腔静脉内径等等)。对采集到的数据进行预处理,即对原始数据进行离散化处理并转化为适合挖掘的事务数据库形式,采用数据挖掘中的关联规则(Apriori算法)对心脏构型的超声数据和相关指标以及各相关指标的影响因素等进行挖掘分析,提取有效关联规则并优选出能反映超重肥胖人群心脏构型及其变化的特征性指标及相关影响因素。结果:建立正常人与超重肥胖患者的心脏构型超声数据关联规则模型,分析超重肥胖患者心脏构型指标及相关影响因素,显示超重肥胖患者左房腔径、左房面积、左房容积、左室心肌重量等指标倾向于发生改变,且心脏构型改变与年龄、肥胖程度、肥胖病程等指标密切相关。结论:利用数据挖据技术对超重肥胖患者的临床信息及心脏构型常规测量指标进行充分挖据,能够提取有价值的关联信息,为进一步的数据研究工作提供参考,并为辅助临床诊断提供依据。
