初诊2型糖尿病患者血清25-羟维生素D水平与胰岛α细胞及β细胞功能的相关性

【摘要】:目的近年研究发现,维生素D缺乏与糖尿病的发生和发展有一定关系。文中旨在初步探讨初诊2型糖尿病患者血清25-羟维生素D[25(OH)D]水平与胰岛α细胞、β细胞功能的关系。方法选取2015年4月至2015年7月在南京军区南京总医院就诊的初诊2型糖尿病患者116例。采用电化学发光法测定其血清25(OH)D水平。根据不同水平将患者分为3组,即维生素D缺乏组:25(OH)D20ng/m L(n=34);维生素D不足组:20ng/m L≤25(OH)D30ng/m L(n=55);维生素D充足组:25(OH)D≥30 ng/m L(n=27)。所有患者均行馒头餐-胰岛素-C肽释放试验,比较3组患者的空腹血糖、空腹胰岛素、空腹C肽、空腹胰高血糖素、稳态模型胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)、早期相胰岛素分泌指数(△I_(30)/△G_(30))、馒头餐-胰岛素-C肽释放试验30 min内C肽曲线下面积(AUC_(CP30))、180 min内C肽曲线下面积(AUC_(CP180))、葡萄糖曲线下面积(AUC_(PG))和胰高血糖素曲线下面积(AUC_(胰高血糖素))。结果 13组患者空腹血糖、空腹胰岛素、空腹C肽、空腹胰高血糖素、HOMA-IR、HOMA-β、AUC_(胰高血糖素)比较,差异均无统计学意义(P0.05)。23组患者△I_(30)/△G_(30)、AUC_(CP30)、AUC_(CP180)、AUC_(PG)比较,差异均有统计学意义(P0.05);其中缺乏组患者△I_(30)/△G_(30)、AUC_(CP30)、AUC_(CP180)均低于不足组、充足组,而AUC_(PG)(52.57±7.45)则高于不足组(48.18±10.32)、充足组(44.39±9.05),差异有统计学意义(P0.05);不足组患者△I_(30)/△G_(30)、AUC_(CP30)、AUC_(CP180)均低于充足组[(3.07±1.60)vs(4.03±1.81)、(2.11±1.22)vs(3.14±1.93)、(15.18±5.24)vs(20.55±12.97),P0.05],而AUC_(PG)则高于充足组(48.18±10.32 vs 44.39±9.05,P0.05)。3Pearson相关分析结果显示,25(OH)D与△I_(30)/△G_(30)、AUC_(CP30)、AUC_(CP180)呈正相关(r值分别为0.4、0.268、0.255,P0.01),与AUC_(PG)呈负相关(r=-0.403,P0.05);偏相关分析结果显示,进一步校正年龄、性别、BMI、收缩压、舒张压、总胆固醇、三酰甘油、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇后,25(OH)D依然与△I_(30)/△G_(30)、AUC_(CP30)、AUC_(CP180)正相关(r值分别为0.416、0.292、0.274,P0.01);与AUC_(PG)呈显著负相关(r=-0.39,P0.01);Pearson相关分析及偏相关分析结果均显示25(OH)D与HOMA-IR、HOMA-β、空腹胰高血糖素、AUC_(胰高血糖素)无显著相关性(P0.05)。结论初诊2型糖尿病患者25-羟维生素D缺乏与早期相胰岛素分泌和总体胰岛素分泌下降有关,与基础胰岛素分泌无关。25-羟维生素D水平与2型糖尿病患者胰岛α细胞功能无明显相关性。
