STOP-Bang问卷在代谢综合征患者中筛查阻塞性睡眠呼吸暂停的应用价值

【摘要】:目的:评价STOP-Bang问卷在代谢综合征(MS)患者中预测阻塞性睡眠呼吸暂停(OSA)的能力。方法:分析2013年12月至2014年5月在新疆医科大学第一附属医院睡眠监测室行睡眠监测(PSG)的317例MS患者的STOP-Bang问卷及PSG监测结果。根据PSG结果将患者分为非OSA组及OSA组(轻、中、重度OSA组),比较各组间性别、STOP-Bang评分及低通气指数(AHI)等结果的差异。分析问卷与PSG监测指标间相关性。应用四格表分析STOP-Bang问卷在不同AHI分界点下的灵敏度、特异度、阳性预测值、阴性预测值。绘制受试者工作特征(ROC)曲线。结果:非OSA组与OSA组性别构成比、年龄、颈围、腹围、收缩压、舒张压、BMI、 HDL-c差异有统计学意义(P均0.01)。非OSA组与其他各组的STOP-Bang评分结果差异均有统计学意义(P0.001)。四组STOP-Bang评分随严重程度增加呈逐渐增高趋势(2.30±1.01,3.48+1.30,3.82±1.26,4.57±1.43)。以3分为截点筛查入选的MS患者是否患OSA,灵敏度为86.36%,特异度为50.94%。以4分为截点预测是否患OSA、中重度OSA、重度OSA, Youden指数均最高。当AHI≥5, AHI15, AHI30时,ROC曲线下面积分别为0.824,0.851,0.892,P均0.001。结论:STOP-Bang问卷在MS人群中应用具有良好的可行性和准确性,值得推广使用。
[Abstract]:Objective: to evaluate the ability of STOP-Bang questionnaire to predict obstructive sleep apnea (OSA) in patients with metabolic syndrome (MS). Methods: from December 2013 to May 2014, 317 cases of MS patients who received sleep monitoring in sleep monitoring room of the first affiliated Hospital of Xinjiang Medical University were analyzed STOP-Bang questionnaire and PSG monitoring results. According to the results of PSG, the patients were divided into non-OSA group and OSA group (mild, moderate and severe OSA group). The differences of sex, STOP-Bang score and (AHI) were compared. The correlation between questionnaire and PSG monitoring index was analyzed. The sensitivity, specificity, positive predictive value and negative predictive value of STOP-Bang questionnaire at different AHI boundary points were analyzed by four-grid table. The (ROC) curves of the operating characteristics of the subjects were drawn. Results: there were significant differences in sex composition ratio, age, neck circumference, abdominal circumference, systolic blood pressure, diastolic blood pressure and BMI, HDL-c between non OSA group and OSA group (all P 0.01). There was significant difference in STOP-Bang score between non-OSA group and other groups (P 0. 001). The STOP-Bang scores of the four groups increased gradually with the increase of severity (2.30 卤1.01 卤3.48 1.30 卤3.82 卤1.26 卤4.57 卤1.43).
