原发性干燥综合征γδT细胞Vδ2亚群表型及与疾病活动程度的相关性

【摘要】:目的通过流式细胞技术检测外周血γδT细胞Vδ2亚群的比例和功能表型,研究原发性干燥综合征(primary Sjogren's syndrome,PSS)患者γδT细胞Vδ2细胞亚群数量及功能变化,及其与疾病活动程度的相关性。方法研究对象分为PSS患者27例,疾病对照组原发性胆汁性肝硬化(primary biliary cirrhosis,PBC)患者10例,健康对照(heal thy control,HC)15例,分别通过流式细胞技术检测其外周血γδT细胞Vδ2亚群占CD3+淋巴细胞和γδT细胞的比例,并检测Vδ2细胞活化、杀伤功能、细胞增殖、细胞凋亡相关功能表型,比较疾病组和对照组之间各指标是否存在显著差异。最后,结合患者临床资料对PSS患者通过疾病活动评估指数(ESSDAI)进行评分,研究V82细胞比例与疾病活动程度的相关性。结果PSS患者组、DC组、HC组Vδ2细胞占CD3+淋巴细胞比例分别为2.152±0.292%,3.790±0.494%,4.900±0.811%,PSS患者组较HC组、DC组均显著降低(P值分别为0.0005,0.0117),而DC组较HC组无明显变化(P值为0.3244)。PSS患者组、DC组、HC组Vδ2细胞占γδT细胞比例分别为60.38±3.930%,75.79±4.717%,78.75±3.719%,PSS患者组较HC组和DC组显著降低(P值为0.0045,0.0199),而DC组较HC组无明显变化(P值为0.6279)。Vδ2细胞表达CD27、CD38、CD69、CD62L、CD45RA、CD45RO、HLA-DR、肿瘤坏死因子-α(TNF-α)、干扰素-r(IFN-r)、颗粒酶(Granzyme A/B)和穿孔素(perforin)、Ki-67、7-AAD等细胞活化、功能、增殖相关分子,且与健康对照组无明显差异。而PSS患者组凋亡相关分子Annexin表达显著增加。PSS患者外周血淋巴细胞中Vδ2细胞所占比例与基于ESSDAI评分的疾病活动程度呈线性负相关。结论与HC和DC组相比,PSS患者外周血Vδ2细胞占CD3+淋巴细胞和γδT细胞的比例明显下降。Vδ2细胞占CD3+淋巴细胞比例与基于ESSDAI评分的疾病活动程度呈线性负相关。HC组相比,PSS患者Vδ2细胞除凋亡相关分子Annexin Ⅴ表达显著增加外,其他细胞功能表型表达无明显变化。
