HIV抗原抗体联检电化学发光法反应性结果与蛋白印迹法检测结果的相关性探讨

【摘要】：目的对比分析艾滋病病毒(HIV)抗原抗体联检电化学发光法(ECL)反应性结果与蛋白印迹法(WB)确证实验检测结果,找出ECL诊断HIV阳性病人的阈值,探讨HIV检测新策略。方法收集郑州大学第一附属医院2012年4月至2016年2月646例病人复筛ECL及HIV确证试验结果,对23例WB不确定结果进行随访确诊,回顾性分析两组检验资料;采用受试者工作特征曲线(ROC)评估ECL检测HIV抗体的能力。结果 ECL检测方法的总阳性率为59.44%(384/646),ECL诊断HIV抗体的ROC曲线下面积为0.990,cutoff值32.17,灵敏度97.10%,特异度96.20%;以cutoff值为界值,COI≥32.17的病人中97.14%为真阳性,COI≥200时,确证实验阳性率为100%,COI32.17的病人中96.23%为假阳性。结论 ECL检测值COI越大,WB的阳性率越高,该结果可为采用酶联免疫试验检测HIV抗体的检测策略提供重要的参考依据。
[Abstract]:Objective to compare and analyze the results of (ECL) reactivity of (HIV) antigen-antibody and Western blot (WB), to find out the threshold value of ECL in the diagnosis of HIV positive patients, and to explore a new method for HIV detection. Methods from April 2012 to February 2016, 646 patients were confirmed by ECL and HIV in the first affiliated Hospital of Zhengzhou University. 23 cases of WB uncertainty were followed up and the data of the two groups were analyzed retrospectively. The ability of ECL to detect HIV antibodies was evaluated by using the operating characteristic curve (ROC). Results the total positive rate of ECL was 59.44% (384 / 646), ECL).
