格列苯脲在高糖环境下对小鼠胚胎成骨细胞株MC3T3-E1增殖、分化及凋亡的影响

【摘要】:背景和目的近年来磺脲类降糖药对骨质疏松的影响引起越来越多学者的关注。多项大型临床研究发现,应用不同降糖药物的患者骨折的发生率有显著差异,其中应用噻唑烷二酮类药物者骨折的发生率最高,胰岛素次之,再其次为二甲双胍,骨折发生率最低的是应用磺脲类降糖药的患者[1-3]。也有部分研究发现磺脲类药物与骨折的发生无明显相关性[4-6],最近统计结果显示,服用不同剂量磺脲类药物的患者骨折风险反而增加,显著高于二甲双胍组[7-8]。关于磺脲类降糖药对骨改建的影响,尚未见国外相关研究,而国内马攀等人[9-11]报道:磺脲类降糖药格列美脲可以在高糖环境下通过上调葡萄糖转运蛋白-1(GLUT-1)和葡萄糖转运蛋白-4(GLUT-4)的表达来促进大鼠下颌骨成骨细胞摄取葡萄糖,并通过磷脂酰肌醇3激酶/蛋白激酶B(PI3K/Akt)通路直接促进大鼠成骨细胞的增殖和分化,并且可以提高碱性磷酸酶(ALP)的活性以及增加I型胶原蛋白(COL-1)和骨钙素(OCN)m RNA表达水平。格列吡嗪和格列喹酮能够促进MC3T3-E1细胞的增殖和I型胶原蛋白的表达,抑制其凋亡[12-13]。格列苯脲作为第二代磺脲类降糖药具有强大的降糖作用,临床应用广泛,且格列苯脲在国外常用于糖尿病合并妊娠的患者。本实验主要探讨不同浓度的格列苯脲在高糖环境下(16.5mmol/l)对体外培养的小鼠胚胎成骨细胞株MC3T3-E1增殖、分化及凋亡的影响。方法经不同浓度格列苯脲干预MC3T3-E1细胞48小时后,CCK-8比色法被用来检测MC3T3-E1细胞的增殖率,应用实时荧光定量PCR(RT-q PCR)检测分化相关标志物I型胶原蛋白(COL-1)和骨桥蛋白(OPN)的表达水平。流式细胞术被用来检测细胞的凋亡率,利用Western blot法检测凋亡蛋白(Bax)和抗凋亡蛋白(Bcl-2)的表达。结果1.CCK-8比色法与对照组相比,随着格列苯脲浓度的升高,MC3T3-E1细胞的增殖率逐渐上升,即1μmol/l格列苯脲组,10μmol/l格列苯脲组和20μmol/l格列苯脲组与对照组相比增殖率依次增加了13.1%、24.2%和31.3%,且差异均具有统计学意义(P0.05)。2.实时荧光定量PCR研究显示,与对照组相比,COL-1、OPN m RNA的表达量随着一定范围内格列苯脲浓度(1μmol/l,10μmol/l,20μmol/l)的升高而增加(P0.05)。3.流式细胞检测术MC3T3-E1细胞的早期凋亡率随着格列苯脲浓度的升高而逐渐下降,即对照组、1μmol/l格列苯脲组,10μmol/l格列苯脲组,20μmol/l格列苯脲组的早期凋亡率依次为19.63%,11.20%,7.9%,4.7%,各组相比差异均具有统计学意义(P0.05)。4.Western Blot与对照组相比,随着格列苯脲浓度的增加,Bcl-2蛋白逐步高表达,Bax蛋白的表达逐步下降,即Bcl-2/Bax的值呈逐步增大的趋势(P0.05)。结论格列苯脲能够在高糖环境下促进MC3T3-E1细胞的增殖和分化,并抑制其凋亡,且在一定范围内,具有浓度依赖性。
