SUDOSCAN检测糖尿病周围神经病变的意义及临床评估

【摘要】:目的通过对比糖尿病周围神经病变的临床诊断标准,评估SUDOSCAN在2型糖尿病远端对称性多发性神经病变的诊断价值。方法实验组:选取2015年8月~2016年7月在青岛大学附属医院内分泌科住院的2型糖尿病患者130例,根据糖尿病远端对称性多发性神经病变的诊断标准将其分为两组:周围神经病变组50例(DPN组)和无周围神经病变组80例(NDPN组),对照组:选取青岛大学附属医院体检中心查体的健康志愿者80例(NC组)。采用SUDOSCAN检测仪对所有受试者进行泌汗神经功能检查,观察3组双手及双足平均电化学皮肤电导率(ESC,单位为μS),采用单因素方差分析比较各组织间手足ESC的差异性,与此同时对感觉神经传导速度与手足ESC进行相关性分析,将手足ESC与糖尿病病程进行相关分析,并将年龄与糖尿病病程进行相关性分析。结果(1)所有受试者在进行SUDOSCAN检测过程中无任何不适感觉,无手掌及足底发红,所有受试者在进行该检查时没有任何不良事件发生。(2)NC组与2型糖尿病患者手平均ESC值分别为(79.1±10.4)μS和(59.7±18.1)μS;足平均ESC值分别为(82.0±8.2)μS和(62.1±21.8)μS,两组之间比较差异均有统计学意义(P0.05)。(3)DPN组、NDPN组手平均ESC值分别为(53.2±18.9)μS和(63.7±16.5)μS;足平均ESC值分别为(53.5±24.4)μS和(67.4±18.2)μS,两组之间比较差异均有统计学意义(均P0.05);(4)所有受试者的手平均ESC值与感觉神经传导速度(SCV)结果均呈正相关,相关系数为r=0.425,(P0.01);所有受试者的足平均ESC值与感觉神经传导速度(SCV)结果均呈正相关,相关系数r=0.445(P0.01)。(5)糖尿病患者的手平均ESC与病程呈明显负相关,相关系数为r=-0.350,(P0.01),足平均ESC与病程亦呈现明显负相关,相关系数为r=-0.428,(r=-0.428,P0.01);(6)糖尿病患者的病程与年龄呈正相关,相关系数为r=0.330,P0.01;(7)手ESC诊断远端对称性糖尿病神经病变ROC曲线下面积分别为0.785,截断值为64.5μS,敏感度为72.0%,特异度为73.7%;足ESC诊断远端对称性糖尿病神经病变ROC曲线下面积分别为0.768,截断值为60.5μS,敏感度为60.0%,特异度为83.7%(均P0.01)。结论(1)SUDOSCAN检测结果与糖尿病病程密切相关,糖尿病病程越长泌汗神经受损伤程度越重,SUDOSCAN检测的手足ESC值也就越低。(2)SUDOSCAN检测的方法简便易行、结果量化、与传统糖尿病神经病变检查(感觉神经传导速度检查)结果具有较高的一致性。(3)SUDOSCAN对于2型糖尿病对称多发性神经病变具有一定的诊断价值,可用于中国人群糖尿病神经病变的诊断。
[Abstract]:Objective to evaluate the diagnostic value of SUDOSCAN in type 2 diabetes mellitus with symmetrical polyneuropathy by comparing the clinical diagnostic criteria of diabetic peripheral neuropathy. Methods: from August 2015 to July 2016, 130 patients with type 2 diabetes mellitus were enrolled in the Department of Endocrinology, affiliated Hospital of Qingdao University. According to the diagnostic criteria of diabetic distal symmetrical polyneuropathy, they were divided into two groups: peripheral neuropathy group (n = 50, DPN group) and non-peripheral neuropathy group (n = 80, NDPN group). Control group: 80 healthy volunteers (NC group) were selected from the physical examination center of affiliated Hospital of Qingdao University. The perspiration nerve function of all subjects was examined by SUDOSCAN detector. The average electrochemical skin conductivity of hands and feet was observed in three groups (ESC, unit 渭 S),). The difference of ESC between hands and feet was compared by single factor variance analysis. At the same time, the correlation between sensory nerve conduction velocity and hand and foot ESC was analyzed, and the correlation between hand and foot ESC and the course of diabetes was analyzed, and the correlation between age and the course of diabetes was analyzed. Results (1) there was no discomfort, no redness of palm and foot in all subjects during SUDOSCAN test. No adverse events occurred in all subjects.
