错配修复基因hMLH1、hMSH2与p53、Rb蛋白在类锐湿疣中的表达

【摘要】:背景与目的:人类错配修复基因作为一组高度保守的看家基因,修复DNA损伤及维持信息传递保真度是它的主要功能。人类错配修复基因家族中最重要的两个基因是hMLH1和]hMSH2,在维持DNA复制保真性和控制基因突变两方面起着重要的作用。有研究表明:hMLH1和hMSH2蛋白表达缺失引起的功能缺陷与HPV相关肿瘤如大肠癌、子宫癌、宫颈癌等的发生密切相关。但hMLH1和hMSH2蛋白是否在尖锐湿疣中有异常表达仍不清楚,本研究拟通过检测hMLH1、hMSH2及p53、Rb蛋白在尖锐湿疣、皮肤鳞癌及正常皮肤中的表达,探讨hMLH1、hMSH2及p53、Rb与尖锐湿疣的发生发展的关系。 材料与方法:诊断明确的尖锐湿疣手术切除标本60例、取皮肤鳞癌20例、正常皮肤10例作为对照组,免疫组化测hMLH1、hMSH2和p53、Rb蛋白在尖锐湿疣和皮肤鳞癌中的表达,并分析尖锐湿疣中hMLH1、hMSH2蛋白异常表达与p53、Rb蛋白表达的关系,所有数据均应用SPSS17.0统计软件分析。 结果:1. hMLH1、hMSH2的蛋白表达情况:hMLH1、hMSH2在正常皮肤组、尖锐湿疣组、皮肤鳞癌组中表达的阳性率分别为90%,70%,40%和100%,80%,35%,其中尖锐湿疣组的表达率高于皮肤鳞癌组(P0.05),且三者之间差别有统计学意义(P0.05)。 2. hMLH1、hMSH2的表达率与尖锐湿疣的组织类型、年龄及性别差别无关(P0.05)。 3.p53蛋白的表达情况:p53在正常皮肤组、尖锐湿疣组及皮肤鳞癌组中表达的阳性率分别为10%,65%和55%,除了正常皮肤组表达的阳性率极低外,尖锐湿疣组和皮肤鳞癌组表达的阳性率均有升高,且三组差别有统计学意义(P0.05)。 4.Rb蛋白的表达情况;Rb在正常皮肤组、尖锐湿疣组和皮肤鳞癌组中表达的阳性率分别为40%,57.3%和70%,三者之间差别无统计学意义(P0.05)。 5.光密度分析:hMLH1,hMSH2与Rb在尖锐湿疣组与鳞癌组中的光密度有差别,分别为(t=4.941,P0.05;t=4.229,P0.05;t=2.643,P0.05),p53在尖锐湿疣与皮肤鳞癌组中光密度无统计学意义(t=-1.576,P0.05)。 6.在尖锐湿疣中各指标之间的相关性分析:hMLH1与hMSH2,hMLH1与Rb呈正相关(r=0.460、r=0.803,P0.05),hMLH1与与p53呈负相关(r=-0.15,P0.05);MSH2与Rb呈正相关(r=0.319,P0.05)与p53呈负相关(r=-0.416,P0.05);Rb与p53则呈负相关(r=-0.120,P0.05)。 结论 1.与正常皮肤组相比,尖锐湿疣组中的]hMLH1、hMSH2蛋白表达减少,与皮肤鳞癌组相比,尖锐湿疣组中的hMLH1、hMSH2蛋白表达增多,提示这二种蛋白可能与尖锐湿疣的发生、发展存在一定的相关性。 2. hMLH1、hMSH2在尖锐湿疣组出现缺失及突变,推测在尖锐湿疣阶段已经有MMR的突变或功能异常;皮肤鳞癌组的突变率升高,可能是由于皮肤鳞癌组织中hMLH1、hMSH2自身发生突变后编码的产物功能下降所造成的无功能性蛋白表达增高,从而增加其缺失和突变率。 3.在正常皮肤组中p53蛋白无阳性表达,而在尖锐湿疣组与皮肤鳞癌组中,p53蛋白阳性表达率较高,提示p53可能与尖锐湿疣和肿瘤的发生有关。 4.Rb蛋白在正常皮肤组、尖锐湿疣组和皮肤鳞癌组中的表达虽均有一定的阳性率表达,但三者差异无统计学意义,提示Rb途径可能并非尖锐湿疣发生的主要途径。 5.在尖锐湿疣组织中,hMLH1、hMSH2与p53关系密切,p53可能是通过上调来修复受损DNA基因。
[Abstract]:BACKGROUND & OBJECTIVE : The human mismatch repair gene plays an important role in repairing DNA damage and maintaining the fidelity of information transmission as a group of highly conserved genes . The most important genes in human mismatch repair gene family play an important role in maintaining the fidelity of DNA replication and controlling the gene mutation .
Materials and Methods : 60 cases were removed from the surgical specimens of Condyloma Acuminatum , 20 cases of skin squamous cell carcinoma and 10 cases of normal skin were used as control group . The expression of hMsh2 and p53 and Rb protein in Condyloma Acuminatum and skin squamous cell carcinoma were detected by immunohistochemistry . All the data were analyzed by SPSS 17.0 statistical software .
Results : 1 . The expression of hMsh2 protein was 90 % , 70 % , 40 % and 100 % , 80 % and 35 % respectively in normal skin group , Condyloma Acuminatum group and skin squamous cell carcinoma group .
2 . The expression rate of hMsh2 was not related to the type of tissue , age and sex difference of Condyloma Acuminatum ( P0.05 ) .
3 . The positive rate of p53 protein expression was 10 % , 65 % and 55 % in normal skin group , Condyloma Acuminatum group and skin squamous cell carcinoma group . The positive rate of p53 protein expression in normal skin group was significantly lower than that in normal skin group ( P0.05 ) .
4 . The positive rates of Rb protein expression were 40 % , 57.3 % and 70 % in normal skin group , Condyloma Acuminatum and skin squamous cell carcinoma , respectively . There was no significant difference among them ( P0.05 ) .
5 . Optical density analysis : The difference of the optical density between the group of patients with Condyloma Acuminatum and squamous cell carcinoma ( t = 4.941 , P0.05 ) .
