2011年吉林省甲真菌病流行病学分析

【摘要】:目的: 了解吉林省甲真菌病病原菌的种类和构成,以便指导临床采取针对性的治疗、提高疗效。 方法: 对自2011年3月至2012年2月,在吉林大学第二医院皮肤科门诊就诊的100例具有典型甲真菌病临床表现,且镜检阳性患者的临床资料及真菌培养结果进行分析。并应用SPSS统计软件进行分析比较。 结果: 临床分型:远端侧位甲下型病甲(DLSO)203个,占60.1%,全甲损毁型病甲(TDO)89个,占26.3%,白色浅表型病甲(SWO)39个,占11.5%,近端甲下型病甲(PSO)7个,占2.1%。共分离培养出致病真菌51株,皮肤癣菌28株(占54.9%),其中红色毛癣菌26株(占51%),酵母菌22株(占43.1%),Geomyces sp.1株占(2%)。 结论: 1、吉林省甲真菌病病原菌主要为皮肤癣菌,其中红色毛癣菌最多见,酵母菌比例较吉林省之前调查结果明显增高。 2、吉林省甲真菌病临床类型以远端侧位甲下型甲真菌病最为多见,全甲损毁型甲真菌病次之。 3、趾甲发病率明显高于指甲的发病率。 4、拇指病甲、拇趾病甲发病率明显高于其它甲。 5、发病年龄以中青年较为多见,男女发病无明显差异。 6、经北京大学第一医院真菌研究中心鉴定,,此次调查中发现一新的菌株Geomyces sp.,关于其致病性尚需进一步研究。
[Abstract]:Objective: To understand the species and composition of the pathogenic bacteria of onychomycosis in Jilin province in order to guide the clinical treatment and improve the curative effect. Methods: From March 2011 to February 2012, 100 patients with typical onychomycosis in Department of Dermatology, second Hospital of Jilin University were studied. The clinical data and fungal culture results of positive patients were analyzed. And the SPSS statistical software was used to analyze and compare. Results: Clinical classification: DLSO (60.1%), TDO9 (26.3cm), SWO (39 cases), PSO (2.7%) and PSO7 (2.1%) in distal lateral hypothyroidism (DLSO) and proximal hypothyroidism (PSO7), with a total of 26. 3% (26. 3%), 39 (11. 5%) in superficial white armour, and 7 (2. 1%) in proximal hypothyroidism (PSO). A total of 51 strains of pathogenic fungi were isolated, 28 strains of dermatophytes (54.9%), 26 strains of Trichophyton rubrum (51%) and 22 strains of yeast (43.1%) were isolated and cultured. Conclusion: 1. The main pathogenic bacteria of onychomycosis in Jilin Province were dermatophytes, among which Trichophyton rubrum was the most common, and the proportion of yeast was higher than that of Jilin Province. 2. The most common clinical types of onychomycosis in Jilin Province were distal subnail onychomycosis, followed by total nail lesion onychomycosis. 3. The incidence of toenail was significantly higher than that of nail. 4. The incidence of thumb and hallucinosis was significantly higher than that of others. 5. The age of onset was more common in young and middle age, and there was no significant difference between male and female. 6. A new strain of Geomyces sp. was identified by the Fungi Research Center of the first Hospital of Peking University. The pathogenicity of the strain remains to be further studied.
