成人跖疣HPV型的检测及与临床特征关系的研究

【摘要】：目的检测感染成人跖疣患者的人乳头瘤病毒(human papillomavirus,HPV)型,了解中国成人跖疣患者中HPV型的分布并探讨其与临床特征之间的关系。方法采用PCR(Polymerase Chain Reaction,PCR)技术检测221例成人跖疣患者HPV型;使用TA克隆(TA cloning)载体技术对混合感染的病毒进行分型;记录患者的年龄、病程、症状和疣体数,分析HPV型与患者临床特征之间的关联性。结果检测标本的阳性率为97.3%(215/221)。本研究总共检测出3个“属”和5个“种”(α“属”第4和8“种”,γ“属”第1“种”,μ“属”第1和2“种”),共7个“型”(HPV 27、1、2、57、63、65和7)。单一HPV型感染患者占96%(206/215),其型别有HPV 27、2、57和7(Alpha属)、HPV-65(Gamma属)和HPV 1、63(Mu属),分别占44.7%、12.1%、7.3%、0.5%、0.5%、33.5%和1.5%。混合型感染患者占4.0%(9/215),混合型别有HPV-1和27、HPV-1和2、HPV-2和27、HPV-1、2和27,分别占55.6%(5/9)、22.2%(2/9)、11.1%(1/9)和11.1%(1/9),HPV-1/27在混合感染中最多见。单一型感染型别构成与按照比例权重分布法计算的型别构成相似。22例镶嵌疣均为单一型感染,所感染的型别有HPV-27、2、1、7、57和63,分别占50.0%(11/22)、22.7%(5/22)、13.6%(3/22)、4.5%(1/22)、4.5%(1/22)和4.5%(1/22)。HPV 2、27和57三种型别感染跖疣患者之间的中位年龄、病程、疣体个数、疼痛的发生率和性别比均无差异(P值0.05)。HPV 27感染者具有年龄30岁、病程1年、疣体个数2个、疼痛发生率低少见的特点,与α属感染者相似。HPV 1感染者与年龄≤30岁、病程≤1年、疣体数≤2个和疼痛高发生率相关,与μ属感染者基本相同。结论HPV 27、2、57和1是感染成人跖疣患者的主要HPV型。HPV 1和27型感染者的临床特征存在很大差异。跖疣相关HPV型与患者的临床特征之间存在着紧密的相关性。
[Abstract]:Objective to investigate the distribution of human papillomavirus (HPV) in Chinese adults with metatarsal warts and its relationship with clinical features. Methods the HPV type of 221 adult patients with metatarsal warts was detected by PCR Polymerase chain reaction PCR, TA clone TA cloning-based vector technique was used to type the mixed infection virus, and the age, course, symptoms and warts of the patients were recorded. To analyze the relationship between HPV type and clinical features of patients. Results the positive rate of the tested specimens was 97.3 / 221%. A total of 3 "genera" and 5 "species" ("4th and 8th" species "of 伪" genera "," 1st "species of 纬" genera "and" 1st and 2nd "species of 渭" genera) were detected in this study. There was no difference in the median age, course of disease, number of warts, incidence of pain and sex ratio among the three types of verruca infections (P = 0.05.HPV-27). There was no difference in age (P = 0.05.HPV27) between the three types of warts infected with HPV-277.The incidence of pain and sex ratio was 0.05.HPV27.
