346例中重度寻常型银屑病患者的临床分析

【摘要】：目的通过对346例中重度寻常型银屑病患者进行问卷调查及相关化验检查,了解银屑病发病的一般规律,并探讨代谢综合征与银屑病病程、严重程度及患者年龄的相关性。方法收集2016年3月-2017年1月间在天津市中医药研究院附属医院皮肤科门诊及病房接受治疗的中重度寻常型银屑病患者,以PASI评分法确定疾病的严重程度,然后采用问卷调查及化验检查相结合的方法对患者一般情况及代谢相关指标进行统计填表,调查表中的内容经EXCEL2007软件进行分类整理,所有记录数据经SPSS21.0软件进行统计分析。结果1.一般资料:346例中重度寻常型银屑病患者中男性220例,女性126例,男女之比为1.75:1,男性多于女性;PASI评分最低10.6分,最高37分,平均(17.10±3.728)分,其中10PASI≤15组127人,15PASI≤20组151人,PASI20组68人;银屑病患者以中青年为主,发病高峰集中在35-50岁,男性发病集中在31-40岁,女性发病集中在21-30岁;病程以5年以上居多,并且以反复发作最常见;职业分布上以退休人员,工人及农民所占比例最高,其次为学生;初发季节以春秋季为主,复发或加重常见于冬季;初次发病部位可累及全身,但以头皮多发,其次为肘部和小腿;有86.13%的患者伴有瘙痒症状,以轻中度瘙痒为主;诱发因素中感染(上呼吸道感染)最常见,其次为情志因素;有银屑病家族史者平均首次发病年龄早于无家族史者,对于有家族史的患者,女性患者平均首次发病年龄早于男性患者,经统计学检验,差异均有显著性(p0.05)。2.合并代谢综合征的情况:346例银屑病患者中符合代谢综合征诊断标准的有107人,占30.92%,其中超重或肥胖、高血糖或已确诊糖尿病、血压升高或已确诊高血压、甘油三酯升高和高密度脂蛋白胆固醇降低的发生率分别为47.11%、10.69%、28.32%、37.86%和42.49%;银屑病患者代谢综合征的患病率与患者年龄呈正相关,与银屑病严重程度也呈正相关,经检验,差异均有统计学意义(p0.05),但与银屑病病程无明显相关关系(p0.05)。结论1.银屑病具有遗传倾向,有家族史的患者初发年龄早于无家族史者,而有家族史的患者中女性初发年龄早于男性。2.银屑病患者代谢综合征的患病率明显高于一般人群。3.代谢综合征的患病率与银屑病患者年龄以及病情严重程度之间呈正相关关系。4.代谢综合征的患病率与银屑病病程之间无明显相关关系。
[Abstract]:Objective to investigate the general regularity of psoriasis and the correlation between metabolic syndrome and the course, severity and age of psoriasis by questionnaire and laboratory examination in 346 patients with moderate and severe psoriasis vulgaris.
