肢端和黏膜黑素瘤PD-L1的表达及其临床意义

【摘要】：目的 :探索程序性死亡配体1(programmed death-ligand 1,PD-L1)在肢端和黏膜黑素瘤患者中的表达情况,并探讨其与临床病理特征及预后之间的相关性。方法 :收集109例肢端和79例黏膜亚型黑素瘤患者的临床资料。采用免疫组织化学法检测患者病灶石蜡包埋组织中PD-L1的表达情况。结合患者临床及病理特征、生存时间等临床数据进行相关性分析。结果 :肢端型患者中PD-L1的阳性表达率为24.77%(27/109),PD-L1在进展期(Ⅲ/Ⅳ期)中的阳性表达率明显高于早期(Ⅰ/Ⅱ期)(P=0.006)。黏膜黑素瘤患者中PD-L1的阳性表达率为30.38%(24/79),与临床及病理特征无关;单因素分析中,PD-L1阳性者的无病生存时间明显缩短(P=0.009),但在多因素分析中有缩短的趋势,但差异无统计学意义(P=0.063)。结论 :部分肢端和黏膜黑素瘤患者中PD-L1表达为阳性,这为在上述2种亚型中使用程序性死亡受体-1(programmed death-1,PD-1)单克隆抗体的治疗提供了理论依据。肢端黑素瘤中PD-L1的表达与肿瘤进展相关,黏膜黑素瘤中PD-L1具有一定的预后判定价值。
[Abstract]:Objective: to investigate the expression of programmed death ligand 1 (programmed death-ligand 1 pPD-L1) in patients with extremity and mucosal melanoma, and to explore its correlation with clinicopathological features and prognosis. Methods: the clinical data of 109 cases of extremity melanoma and 79 cases of mucosal subtype melanoma were collected. Immunohistochemical method was used to detect the expression of PD-L1 in paraffin embedded tissues.
