面部皮肤恶性肿瘤综合治疗分析

【摘要】：目的：探讨面部皮肤恶性肿瘤综合治疗的临床疗效及其术后功能形态的恢复,能更好的指导我们的临床实践工作,以手术治疗为主的综合治疗这种较为理想的治疗方式能在临床上能得到积极的推广运用。方法：回顾性分析我科2004年9月至2010年9月的面部恶性肿瘤临床资料72例,基底细胞癌52例,鳞癌16例,其它类型(Bowen病、表浅黑色素瘤、瘢痕癌等)4例,男43例,女29例；年龄30-85岁,以61-70岁多见,平均67岁,其中病灶位于额部12例,眼周23例、鼻部18例,颞部9例,颧颊部7例,口周3例,其中≤30岁1例,31-40岁2例,41-50岁5例,51-60岁14例,61-70岁28例,71-80岁16例,≥81岁以上6例。根据病灶的部位和范围,扩大切除病灶后行直接对位缝合,局部皮瓣转移,皮片移植,局部皮瓣+皮片移植为主的手术方式并术后辅助放疗或化疗的综合治疗。结果：72例研究资料中1年后3例基底细胞癌复发,鳞癌无复发,复查全身CT后发现有1例发生远处转移,其它类型患者3年生存率较好,其余患者皮片或皮瓣全部成活,切口愈合良好,电话随访1-3年,患者术后外形与功能良好,除1例因植皮后有轻度的色素沉着外,其它患者美容效果较好,未引起继发畸形,医患双方对手术效果均感较满意,在本组中发现基底细胞癌组、鳞癌组和其它组在手术方式治疗上无明显的差异性。结论：早期诊断、早期治疗是治疗面部恶性肿瘤的根本所在,面部恶性肿瘤以手术为主的综合治疗有较好的临床效果,可根据患者病灶情况不同满足个体化综合治疗方案,以最佳的修复方式来满足患者的病情,不仅可以从根本上清除病灶,也可兼顾患者术后其功能和形态的完美统一。
[Abstract]:Objective: to explore the clinical effect of comprehensive treatment of facial malignant tumors and the recovery of functional morphology after operation, so as to better guide our clinical practice.
