HBD-2，-3和TNFα在寻常型银屑病皮损中的表达及意义

【摘要】：目的：本实验通过对进行期寻常型银屑病皮损组织、非皮损组织及正常人皮肤组织中防御素-2(HBD-2)、防御素-3(HBD-3)及肿瘤坏死因子（TNF-α）表达情况的研究，探讨HBD-2，-3与TNF-α之间的关系；三者表达水平与银屑病严重程度之间的关系，进一步揭示HBD在寻常型银屑病发生发展过程中的作用机制。方法： 1.收集45例进行期寻常型银屑病患者的皮损组织、非皮损组织及15例正常人皮肤组织,采用实时荧光定量逆转录聚合酶链反应法(RT-PCR)检测标本中HBD-2，-3及TNF-α的mRNA表达水平。比较不同组别间三者表达的差异，分析HBD-2与TNF-α，HBD-3与TNF-α表达的相关性。 2.将45例进行期寻常型银屑病患者按照PASI评分分为轻、中、重三组，，其中轻度组25例，中度组12例，重度组8例。采用RT-PCR检测标本中HBD-2，-3及TNF-α的mRNA表达水平。比较不同组别间三者表达的差异，分析三者与银屑病PASI评分的相关性。结果： 1.HBD-2，-3及TNF-αmRNA在正常对照组、非皮损组及皮损组中均有表达，且表达依次升高。HBD-2、HBD-3及TNF-α的mRNA在皮损组中的表达显著高于非皮损组（P＜0.01）和正常对照组（P＜0.01）；在非皮损组中的表达又显著高于正常对照组（P＜0.01）。 2.在皮损组中HBD-2与TNF-α，HBD-3与TNF-α的表达均呈正相关，相关系数分别为r=0.807、r=0.809；（P值均＜0.05）。 3.HBD-2，-3及TNF-αmRNA在银屑病皮损重度组中的表达显著高于中度组（P＜0.01）和轻度组（P＜0.01）；在中度组中的表达又显著高于轻度组（P＜0.01）。 4.HBD-2、HBD-3及TNF-α的mRNA在皮损组中的表达均与患者PASI评分成正相关，相关系数分别为r=0.887、r=0.756、r=0.785；（P值均＜0.05）。结论：寻常型银屑病皮损中HBD-2，-3及TNF-αmRNA的表达与银屑病PASI评分正相关。HBD-2，-3与TNF-α的表达之间也存在正相关。我们推测三者之间可能存在相互作用相互诱生的关系。HBD-2、HBD-3的表达增高是一把双刃剑在防御感染的同时，也可能扩大了炎反应，与TNF-α一起参与了银屑病的发病过程。
[Abstract]:Objective: to investigate the effects of defensin-2 (HBD-2) in psoriasis vulgaris, noninvasive tissues and normal skin tissues in patients with psoriasis vulgaris. To investigate the expression of defensin-3 (HBD-3) and tumor necrosis factor 伪 (TNF- 伪) and to explore the relationship between HBD-2,-3 and TNF- 伪. The relationship between the three levels of expression and the severity of psoriasis further revealed the role of HBD in the pathogenesis of psoriasis vulgaris. Methods: 1. HBD-2, in 45 patients with psoriasis vulgaris was detected by real-time quantitative reverse transcriptase polymerase chain reaction (RT-PCR). MRNA expression level of-3 and TNF- 伪. The expression of HBD-2 and TNF- 伪, HBD-3 and TNF- 伪 were compared among different groups. 2. According to PASI score, 45 patients with psoriasis vulgaris were divided into mild, moderate and severe groups, including 25 mild, 12 moderate and 8 severe psoriasis. The mRNA expression of HBD-2,-3 and TNF- 伪 was detected by RT-PCR. The differences of expression among different groups were compared and the correlation between the three groups and PASI score of psoriasis was analyzed. Results: 1. HBD-2n-3 and TNF- 伪 mRNA were expressed in normal control group, non-lesion group and skin lesion group, and the expression of HBD-2n-3 and TNF- 伪 mRNA increased in turn. The expression of mRNA of HBD-3 and TNF- 伪 in the lesion group was significantly higher than that in the non-lesion group (P < 0. 01) and the normal control group (P < 0. 01). The expression in non-lesion group was significantly higher than that in normal control group (P < 0. 01). 2. There was a positive correlation between the expression of HBD-2 and TNF- 伪, HBD-3 and TNF- 伪 in the lesion group, and the correlation coefficients were 0.807 and 0.809, respectively (P < 0. 05). 3. The expression of HBD-2n-3 and TNF- 伪 mRNA in severe psoriatic lesions was significantly higher than that in moderate and mild psoriatic lesions (P < 0. 01), and was significantly higher in moderate group than in mild group (P < 0. 01). 4. The expression of HBD-3 and TNF- 伪 mRNA in the lesions was positively correlated with the PASI score, and the correlation coefficients were 0.756 and 0.756, respectively (P < 0. 05). Conclusion: the expression of HBD-2,-3 and TNF- 伪 mRNA in psoriatic lesions is positively correlated with the PASI score of psoriasis, and there is a positive correlation between the expression of HBD-2,-3 and TNF- 伪 in psoriasis vulgaris. We speculate that there may be interaction and interaction between the three. The increased expression of HBD-2,HBD-3 is a double-edged sword that may expand the inflammatory response while defending against infection. TNF- 伪 is involved in the pathogenesis of psoriasis.
