IL-21、CCL20在扁平苔癣皮损中的表达及意义

【摘要】:目的:探讨扁平苔癣皮损中白介素-21(IL-21)和CC趋化因子配体20(CCL20)的表达及意义。方法:病例组:30例,均为2015年-2016年就诊于山大一院皮肤科门诊,并且经临床和病理确诊为扁平苔藓的患者,其中男16例,女14例,年龄18-60岁,平均年龄41.73岁,半年内均未接受过糖皮质激素、免疫抑制剂、免疫调节剂治疗,且排除其他全身系统性疾病、严重肝病、肾病、自身免疫性疾病。对照组:30例,均为同期就诊于山大一院整形科的健康人,其中男13例,女17例,年龄20-65岁,平均年龄43.3岁,均无任何皮肤病和系统性疾病。两组成员的性别和年龄比较差异均无统计学意义(P0.05),具有可比性。所有组织均经甲醛固定石蜡包埋备用,IL-21、CCL20在扁平苔藓皮损中的表达水平采用免疫组织化学方法测定。运用SPSS17.0软件来对最后的实验结果进行分析,采用两独立样本t检验的方法来比较两组,Pearson相关分析用来分析指标间的相关性,差异有统计学意义的评判标准为P0.05。结果:IL-21和CCL20的表达水平均用灰度值大小来表示。IL-21在扁平苔藓患者皮损中的表达水平为(189.19±15.42),在健康对照组中的表达水平为(232.55±12.97)。CCL20在扁平苔藓皮损中的表达为(198.91±18.65),在健康对照组的表达水平为(225.68±8.72)。统计分析结果显示,IL-21、CCL20在扁平苔藓患者皮损中的表达水平较健康对照组升高(t分别为10.532,6.112),差异比较均有统计学意义(均P0.001)。Pearson相关分析表明在扁平苔藓患者皮损中IL-21、CCL20的表达成正相关(r=0.691,P0.001)。结论:IL-21、CCL20在扁平苔藓患者皮损中的表达水平高于健康对照组(P0.001),且二者在扁平苔藓皮损中的表达水平成正相关性(P0.001),提示IL-21、CCL20可能在扁平苔藓的发病中发挥一定的作用,同时可能为扁平苔藓的治疗提供了新的理论依据。
[Abstract]:Aim: to investigate the expression and significance of interleukin-21 (IL-21) and CC chemokine ligand 20 (CCL20) in the lesions of lichen planus. Methods: a total of 30 patients with lichen planus, including 16 males and 14 females, aged 18-60 years, with an average age of 41.73 years, were admitted to the dermatology clinic of the first Hospital of Shandong Province from 2015 to 2016. No glucocorticoids, immunosuppressants, immunomodulators were used in six months, and other systemic diseases, severe liver diseases, nephropathy, autoimmune diseases were excluded. The control group consisted of 30 healthy persons, including 13 males and 17 females, aged 20-65 years, with an average age of 43.3 years, all of them without any skin diseases and systemic diseases. There was no significant difference in sex and age between the two groups (P0.05), which was comparable. The expression of IL-21,CCL20 in lichen planus was determined by immunohistochemical method. SPSS17.0 software was used to analyze the final experimental results, and two independent samples t-test method was used to compare the two groups. Pearson correlation analysis was used to analyze the correlation between the indicators. The difference was statistically significant (P0.05). Results: the expression levels of IL-21 and CCL20 were expressed in gray scale. The expression level of IL-21 in the lesions of lichen planus was (189.19 卤15.42). The expression of CCL20 was (198.91 卤18.65) in lichen planus and (225.68 卤8.72) in healthy control. The results of statistical analysis showed that the expression of IL-21,CCL20 in the lesions of lichen planus patients was higher than that in healthy controls (t = 10.532 卤6.112, respectively). The difference was statistically significant (P0. 001). Pearson correlation analysis showed that the expression of IL-21,CCL20 was positively correlated in the lesions of patients with lichen planus (r 0. 691P 0. 001). Conclusion: the expression of IL-21,CCL20 in patients with lichen planus is higher than that in healthy controls (P0. 001), and there is a positive correlation between the expression of IL-21,CCL20 and Lichen planus (P0. 001), which suggests that IL-21, is expressed in patients with Lichen planus (P0. 001). CCL20 may play a role in the pathogenesis of lichen planus and may provide a new theoretical basis for the treatment of lichen planus.
