自体毛发单体毛囊移植术(FUT)在永久性秃发的治疗中的应用

【摘要】: 目的:雄源性秃发、瘢痕性秃发等均引起毛囊的部分或全部萎缩、坏死导致永久性毛发脱落,是一种不可逆性秃发,不仅影响患者的外观,对其心理也产生不同程度的阴影。传统的毛发外科经历了头皮缩减术、皮瓣外科、头皮自体移植3个阶段。但传统的方法修复方法大多数容易产生成活率低、疤痕明显、外观不自然等缺陷,我院自2003年9月以来,我们采用单体毛囊自体毛发移植术(FUT)治疗永久性秃发61例,观察其效果。 方法:回顾性分析我院整形外科自2003年9月到2008年3月,采用自体毛发单体毛囊移植术(FUT)治疗61例永久性毛发缺失患者的资料,先在颅后部优势供区内切取含有完整毛发的头皮组织,将其分割成含单体毛囊的移植物,然后将移植物植入头皮缺失区内预制的孔洞中。术后随访满6个月到1年后,进行疗效评定,并随机选10例患者复查毛发成活状况。 结果:大部分患者术后移植的毛发均生长良好,61例患者术后随访6个月至1年,毛发成活率均达到90%以上,修复后的毛发外观接近自然毛发分布特点,患者满意度达到90%,供区瘢痕不明显。 结论:自体毛发单体毛囊移植术(FUT)在永久性脱发的治疗中种植的毛囊单位存活率高,修复后毛发外观自然,供区瘢痕不明显,是治疗雄源性及瘢痕性永久性秃发的有效手段。
[Abstract]:Aim: male alopecia and cicatricial alopecia all cause partial or total atrophy of hair follicles, necrosis leads to permanent hair shedding, which is irreversible alopecia, which not only affects the appearance of patients, but also has different degree of shadow on their psychology. Traditional hair surgery has undergone three stages: scalp reduction, flap surgery and scalp autotransplantation. However, most of the traditional methods are easy to produce defects such as low survival rate, obvious scar, unnatural appearance and so on. Since September 2003, 61 cases of permanent alopecia have been treated with single hair follicle autotransplantation (FUT). Observe its effect. Methods: from September 2003 to March 2008, 61 patients with permanent hair loss were treated with autologous hair follicle transplantation (FUT). First, the scalp tissue containing complete hair was removed from the superior donor region of the posterior skull and divided into grafts containing monomeric hair follicles. Then the grafts were implanted into prefabricated holes in the missing areas of the scalp. After 6 months to 1 year follow-up, the curative effect was evaluated, and 10 patients were randomly selected to review the survival status of hair. Results: most of the patients had good hair growth after transplantation. After 6 months to 1 year follow-up in 61 patients, the survival rate of hair reached more than 90%, and the hair appearance after repair was close to the distribution of natural hair. The satisfaction rate of patients reached 90%, and the scar in donor area was not obvious. Conclusion: the survival rate of hair follicles implanted by (FUT) in permanent alopecia is high, the hair appearance is natural after repair, and the scar in donor area is not obvious. It is an effective method to treat male and cicatricial permanent alopecia.
