动静脉溶栓治疗缺血性脑卒中的临床疗效观察

【摘要】：目的观察动静脉溶栓治疗缺血性脑卒中的临床疗效。方法选取河北工程大学附属医院2011年1月—2014年1月收治的缺血性脑卒中患者111例,根据治疗方法分为动静脉溶栓组59例与动脉溶栓组52例。在常规对症治疗基础上,动静脉溶栓组患者采用动静脉溶栓治疗,动脉溶栓组患者采用动脉溶栓治疗。比较两组患者临床疗效、血管再通情况、并发症发生情况及死亡情况。结果动静脉溶栓组患者临床疗效优于动脉溶栓组(u=-4.249,P=0.000)。动静脉溶栓组患者血管再通情况优于动脉溶栓组(u=-2.399,P=0.016)。动静脉溶栓组患者病灶内渗血及病死率低于动脉溶栓组(P0.05),而两组患者牙龈出血、颅内出血、消化道出血发生率比较,差异无统计学意义(P0.05)。结论动静脉溶栓治疗缺血性脑卒中的临床疗效确切,可有效改善患者神经功能及促进患者血管再通,且未增加出血事件发生风险,安全性较高。
[Abstract]:Objective to observe the clinical effect of arteriovenous thrombolytic therapy on ischemic stroke. Methods 111 patients with ischemic stroke were selected from January 2011 to January 2014 in the affiliated Hospital of Hebei University of Engineering. According to the treatment methods, 59 patients in the arteriovenous thrombolysis group and 52 patients in the arterial thrombolysis group were divided into two groups. On the basis of routine treatment, the patients in the arteriovenous thrombolytic group were treated with arteriovenous thrombolytic therapy. The patients in the arterial thrombolytic group were treated with arterial thrombolytic therapy. The clinical efficacy and vascular recanalization were compared between the two groups. Results the clinical effect of arteriovenous thrombolytic group was better than that of arterial thrombolytic group. The vascular recanalization in the arteriovenous thrombolytic group was better than that in the arterial thrombolytic group. The incidence of bleeding, intracranial hemorrhage and gastrointestinal bleeding in patients with arteriovenous thrombolysis was lower than that in patients with arterial thrombolytic therapy (P 0.05), while the incidence of gingival bleeding, intracranial hemorrhage and gastrointestinal bleeding was lower in the patients with arteriovenous thrombolysis than that in the patients with arterial thrombolytic therapy. Conclusion Arteriovenous thrombolysis is effective in the treatment of ischemic stroke, which can effectively improve the neurological function of patients and promote the recanalization of blood vessels. The risk of bleeding events was not increased and the safety was high.
